DETAILS
- Department: Inpatient
- Schedule: Monday-Friday, 8:00AM-4:30PM, and one weekend every 6 weeks, 8:00AM-4:30PM
- Hospital: Ascension St. Agnes
- Location: Baltimore, MD
- Salary: $53.76-$80.64
Actual compensation offered will vary based on factors including applicable licenses, certifications, education, experience, location, qualifications and comparison with associates in similar roles.
REQUIREMENTS
Licensure / Certification / Registration:
- Pharmacist credentialed from the Maryland Board of Pharmacy obtained prior to hire date or job transfer date required.
Education:
- Bachelor’s or advanced degree in Pharmacy required.
EDUCATION
- Graduate of an ACPE accredited College of Pharmacy
- Doctor of Pharmacy or other advanced degree highly preferred
EXPERIENCE
- Completion of an ASHP accredited PGY-1 Pharmacy Residency required
- Completion of an ASHP accredited PGY-2 Pediatric Pharmacy Residency is preferred
- Clinical pharmacy experience (2-4 years) in a hospital setting preferred (required if no PGY-2 residency)
- Board of Pharmacy Specialists certification in Pediatric Pharmacy preferred
- Knowledge of contemporary hospital/clinical practice and service
- Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine in pediatrics, neonatal intensive care and maternal health.
Responsibilities:
Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership.
- Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.
- Recommend formulary alternatives when appropriate.
- Compound and dispense medications for accurate delivery to patients.
- Provide drug information and education to medical staff and patients.
- Communicate with physicians and other clinicians as necessary to facilitate care.
- Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.
- Assist with academic functions such as serving as a preceptor for pharmacy students.
- May float to provide coverage of pharmacy needs.
- Provide patient counseling when required.
