EDUCATION AND TRAINING:

  • Minimum of a four (4) year Bachelor’s Degree in Pharmacy, from an accredited university.
  • Doctorate of Pharmacy (PharmD) with a residency program recognized by the OCP is preferred.
  • Current Certificate of Registration in good standing with the OCP.
  • Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

EXPERIENCE:

  • Three (3) years’ experience working in a community Pharmacy is preferred.
  • Experience working in a clinical Oncology Pharmacy environment is preferred.
  • Experience in academic teaching with health science learners is preferred.

Responsibilities:

  • Optimize drug therapy and review orders for accuracy.
  • Provide medication management services (i.e. treatment recommendations, drug information and support to an interdisciplinary team serviced by the North East Cancer Centre (NECC) and HSNRx retail pharmacies).
  • Work with patients/families, caregivers and other health care professionals to provide medication management services (e.g. medication counselling, medication adherence support, etc.).
  • Assist with insurance approval for complex outpatient medication regimens.
  • Respond to requests for drug/therapeutic information from other health care professionals.
  • Provide oral and written follow-up with treatment recommendations to the prescriber as required.
  • Provide medication reconciliation services and interact with patients/families and other health care professionals with regards to patients’ home medication usage.
  • Ensure completion of required documentation in conformance with the standards of practice of the Ontario College of Pharmacists (OCP) and HSN policies.
  • Oversee drug use control of all medications, including security of narcotics and controlled drugs.
  • Ensure verification of all technical work by directly confirming or via the standards of practice of the OCP and HSN policies.
  • Ensure prescribing, dispensing and selling medication and other controlled acts are performed in compliance with the Regulated Health Professionals Act (RHPA), the Drugs and Pharmacies Regulation Act, other relevant Provincial and Federal legislation and HSN policies or procedures.
  • Perform Therapeutic Drug Monitoring for required drugs.
  • Provide drug therapy management education for physicians, nurses, allied health care providers, patients and caregivers.
  • Perform and be an advocate for adverse drug reaction and medication error reporting and meet with interdisciplinary groups specific to patient care areas or organization wide to implement preventative changes for the future.
  • Maintain a safe and clean working environment by complying with health and safety procedures, rules, and regulations; report compliance gaps to manager for follow up.
  • Maintain pharmaceutical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks and participating in professional societies.
  • Complete departmental projects (e.g. formulary reviews, IV monographs, DUE projects, policy and procedure development, research projects).
  • Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  • Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  • Educate and promote health, safety and wellness in the work place.
  • Represent the department or program on various committees and in meetings as required.
  • Perform other duties as required.
