QUALIFICATIONS:

Required:

  • B.S. or Pharm.D. from an ACPE-accredited School of Pharmacy
  • Registered Pharmacist in NJ
  • AHA BLS Certification

Preferred:

  • Completion of a PGY1 residency program or experience equivalent
  • Pediatric pharmacy experience
  • Board Certified in area of pediatric practice or equivalent experience

Responsibilities:

ESSENTIAL FUNCTIONS:

  • Prepares and dispenses medication orders per provider order according to established policies, procedures and protocols
  • Identifies, resolves and prevents drug-related problems
  • Interprets medication orders and transcribes them into the EMR accurately if necessary
  • Maintains accurate and complete patient medication profiles
  • Completes accurate calculations for pediatric drug therapy for compounding, dispensing and drug therapy monitoring
  • Interacts with patients, family, and other health care providers as appropriate to assure continuity of services and to achieve desired patient outcomes through safe, timely, cost-effective drug therapy
  • Identifies and selects specific dosage forms as appropriate for the pediatric population
  • Assures compliance with organizational policy regarding medication management
  • Assures integrity of medication including visual review, adequate storage and expiration management
  • Participates in the prevention of drug diversion per department and facility policy and procedure
  • Ensures universal pharmaceutical waste and hazardous pharmaceutical waste policies and procedures are appropriately completed
  • Successfully utilizes automated dispensing system
  • Performs assigned nursing station inspections every other month as assigned
  • Demonstrates effective direction, supervision and evaluation of technical staff
  • Consistently collaborates with medical and hospital staff to provide an environment which assures the safety and well-being of pediatric patients
  • Accurately reports all patient care or medication errors, adverse events and near misses as soon as possible and documents appropriately
  • Consistently uses error prevention processes, especially those identified with high risk procedures or medication delivery systems
  • Consistently demonstrates knowledge of the chain of command policy and applies in appropriate situations
  • Consistently adheres to all patient safety and medication management policies and procedures
  • Assesses, collaborates, and coordinates the patients discharge planning needs and readiness for discharge with other members of the health care team as necessary
  • Completes mandatory online educational sessions in a timely manner
  • Demonstrates awareness and respect to the patient, family and/or significant other as an individual and family unit (identifies cultural and religious needs, language barriers, physical and psychological limitations, etc.)
  • Ensures timely and accurate communication of patient information with all members of the health care team

Physical Demands

  • Requires prolonged sitting, some bending, stooping and stretching
  • Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipment
  • Requires normal range of hearing and eyesight to record, prepare and communicate reports. Requires occasional lifting of boxes or totes up to 50 pounds
  • May be exposed to toxic/ caustic/chemicals/detergents
  • Work is performed in an office, and in the pharmacy and hospital environment
  • Work may be stressful at times. Contact may involve dealing with angry or upset people
  • Staff must remain flexible and available to provide staffing assistance for any/all disaster or emergency situations

Work Environment:

  • May be subjected to noise and disagreeable odors
  • Moving mechanical parts are found in the work environment
  • Universal precautions must be followed when working in any clinical area and especially when entering patient care areas
  • Chemotherapy and hazardous medication preparation requires some exposure to potentially harmful chemicals and medications. Workers are exposed to chemical hazards and must use appropriate personal protective equipment as trained
  • Sterile product/IV preparation exposes employee to clean needle stick injury
