Description:

UPMC Hamot in Erie, PA is seeking a highly motivated and detail-oriented Medication Reconciliation Pharmacy Ops Specialist, Associate to join our dynamic pharmacy team. This specialist will play a critical role in ensuring accurate medication histories for patients, contributing to improved patient outcomes and safety. Our full-time role will serve 10-hour evening shifts with rotating weekends holidays. Responsibilities:

  • Medication Reconciliation: Conduct comprehensive medication reconciliation for patients at admission, transfer, and discharge. Primary work will be focused on admission medication reconciliations for patient admitting through the emergency department.
  • Patient Interviews: Obtain detailed medication histories through patient interviews, family members, and outpatient pharmacy records.
  • Documentation: Document medication histories accurately in the electronic health record (EHR) system.
  • Collaboration: Work closely with pharmacists, physicians, nurses, and other healthcare professionals to clarify discrepancies and ensure accurate medication information.
  • Education: Provide education to patients and caregivers regarding their medications.
  • Quality Improvement: Participate in quality improvement initiatives related to medication reconciliation processes.
  • Compliance: Ensure compliance with Joint Commission standards and hospital policies regarding medication management.
