Description:

JOB PURPOSE: 1.The post holder will provide expert advice and support on all aspects of medicines optimisation to ensure use of medicines is optimised and meets safety, statutory and governance requirements. In conjunction with the Integrated Care Pharmacist, the post holder will2.Act as first point of contact for any medicines management issues and support all aspects of medicines management within the service; they will work independently to provide a seamless pathway for medication support from in-patient to community services/patient home.3.Support the development of a priortisation matrix, referral pathways in to the medicines optimisation team and undertake structured medication reviews including domiciliary visits when required that work to reduce the risk of harm from taking medicines.4.Carry out and support procedures for medicines reconciliation, medicine adherence and self- administration to ensure pharmaceutical care is appropriate in community care and home settings. Structured medication reviews targeted to those people with the most significant health needs will be undertaken in coordination with acute Trusts, GPs and practice-based clinical pharmacists to reduce overmedication, re-admissions and support meaningful savings in unnecessary prescribing costs5.Support Escalation and Resolution of medication related discrepancies/queries with external stakeholders (acute Trusts/community pharmacies/GP) and ensures that requests are processed safely and prioritised to match clinical urgency. Support reduction in medication escalation to on-call clinical leads6.The post holder will create MARs charts using the Trust MAPPs system, which will be used by the Home Fist Support Workers to record administration of medication to the patient / service user7.Provide professional advice and support to other health and social care professionals on the safe prescribing, handling and administration of medicines working within the limits of their knowledge at all times and referring to a pharmacist where necessary.8.Provide tailored interventions based on an individual level of need and circumstances support re-ablement, promote independence and self-care9.Identify, develop and provide support to meet medicines related training needs for clinical and non-clinical staff to reduce medicines related adverse incidents and improve outcomes10.Develop partnerships and facilitating collaborative working between multidisciplinary teams especially during the transfer of care to optimise medicine-related discharge processes, ensure safe transitions of care and reduce readmissions.
This will include developing links to Care Homes, GP Practices and Primary Care Networks11.Support policy and service development within the service, and be responsive to emerging problems and variations as and when they arise, escalating where necessary.12. The post holder will work as part of the pharmacy team at MPFT to ensure the safe, effective and economical use of medicines, supporting the development of a comprehensive medicines optimisation service to all of our service users.13.The post holder will be expected to be responsible for planning and delivering own workload and agreeing deadlines, to promote safe, legal, rational and economic use of medicines by all users of the pharmacy service.14.Support a learning culture within all health and social care services to ensure continual service improvement
