Description:
ROLE PURPOSE / SUMMARY The Mid and South Essex Integrated Care System comprises the Integrated Care Board (ICB), which is the statutory body for the NHS, and the Integrated Care Partnership (ICP), which is a committee of the ICB, and brings together key health, care, community and voluntary sector organisations across the area. The ICS has a bold ambition to deliver improved outcomes for our 1.2m population through our four Alliances, and we are placing clinical and professional leadership and the voice of our residents at the heart of realising this ambition. Pharmacy and medicines optimisation sits within the Clinical Leadership and Innovation Directorate which aims to put clinical and multi-professional leadership at the heart of our system. The directorate identifies, recruits and retains an inclusive group of motivated clinical leads that helps patients, front-line staff and professional bodies to become closely engaged, and work with Integrated Care System (ICS) governance to support collective accountability.
The post-holder will deliver medicines optimisation to ensure cost-effective, safe medicines use, enabling the best possible health gains for patients. This will be achieved through supporting clinical governance and medicines safety in health and social care. The post-holder will look to empower patients to get the maximum benefit from their medicines while reducing the risk of medication-related harm, focussing on high-risk drugs, complex situations and vulnerable patients. COMPETENCY BASED KEY DELIVERABLES Responsible for decisions made during the course of duties, seeking guidance and escalating issues where appropriate Assist with the delivery of project plans and workstreams, as directed by line manager and raising any issues with delivery to line manager.
Responsible for monitoring budget/s, contributing to budget setting and working knowledge of financial processes. Provide technical support to implementation and delivery of one or more strategic plans, enabling key milestones to be delivered and raising any issues associated with the delivery of plans and workstreams. Provide technical support for medicines governance to maintain medicines safety and quality. Provide technical support with the delivery of the medicines governance agenda Provide technical support to the system where medicines related CQC standards are not met.
Provide technical support to monitoring prescribing of all prescribers, including non-medical prescribers. Responsible for monitoring actions from local national and regional safety alerts in line with ICB process. Provide technical support in monitoring high risk drugs, antimicrobial stewardship and dependence-forming medication. Maintain and utilise Eclipse risk stratification tool to inform medicines governance and medicines safety strategies.
Utilise and support the use of prescribing support tools e.g. Scriptswitch, in relation to preventing prescribing-related harm Provide technical support to Mid and South Essex quality team with investigating medicines safety incidents and implement best practice. Provide technical support to the review and maintenance of safe access to medicines, e.g. repeat dispensing, electronic prescriptions, Patient Group Directions Provide technical support for controlled drugs advice and monitoring, including controlled drugs destruction.
Provide technical support for medicines-related queries and complaints
