Description:

Please refer to the Job Description and Person Specification for full details To provide support and advice for individuals with a LD who also have a physical healthcare condition who are admitted too ULH hospitals. To needs led interventions based on assessed needs which are measured against agreed outcomes, To provide support to ULHT when cover the ALN nurses are on annual leave. To support individuals with learning disabilities to have their health needs met across all health care settings, including planned admission/appointments to hospital. To work with patients who do not attend Annual health check appointments and /or NHS screening programs to establish the reason for the non-attendance and provide advice support to the individual with any reasonable adjustments if appropriate.
To work flexibly to support the service. To provide expert advice and support to clinicians within LPFT and beyond in regard to meeting the physical health care needs of people with a learning disability To work in collaboration with UHLT and primary care to ensure that the individuals with a learning disability are supported utilising the Mental Capacity Act to make decisions relating to their care and treatment. Participate and/or coordinate best interest meetings. The core hours of the role are 9-5pm Monday to Friday but there may be the need on occasions to work flexibly to support the service.
Provide management and clinical supervision as required to peers and junior staff in line with the Trust policies and protocols for supervision, appraisal, and mandatory training. To ensure a high standard or clinical practice and that clinical skills are updated as required. To manage a complex caseload ensuring that records are kept in line with Trust and service expectations
