REQUIRED QUALIFICATIONS:

• Degree in Physical Therapy.
• Upon hire: Washington Physical Therapist License.
• Within 30 days of hire: National Provider BLS - American Heart Association
• Within 30 days of hire: National Provider Identifier (NPI) number
• 1 year Home Health.

PREFERRED QUALIFICATIONS:

• 2 years Physical Therapy with adults with physical disabilities.
• 1 year Experience with the geriatric population.
