. This exciting leadership opportunity has arisen for a Senior Physiotherapist in Camden. Following National and local drivers for change there is a clear vision to integrate statutory and voluntary agencies, maximising their impact by enhancing access to services ,reducing health inequalities. Camden Integrated District Nursing and Community Rehabilitation Therapies are aligned in 5 neighbourhood teams with a vision to mobilise into multiagency co location. The post holder will be based in Belsize Priory Health Centre to manage the West Camden Integrated Neighbourhood District Nursing and Therapies Team whilst also leading on the Camden innovative falls prevention service. Our teams are led by either Senior District Nursing or Therapy staff who work closely together in a leadership team alongside the District Nursing Practice Development Nurse, ensuring a robust clinical supervision matrix across the professional scope of practice. Working in partnership with North Team nursing Lead, the post holder will provide ensure coordination of patient care and deployment of staff to meet the needs of patients.

As the Lead Physiotherapist you will be expected to be a senior therapy lead who is able to adapt to any issues arising from the demands of the commissioned service. You will be part of a community team where your proven ability to make high level clinical decisions will aid patient outcomes and partnership working. The post holder will work as part of the CICH Leadership Team which consists of the Head of CICH, Occupational and Physiotherapy Leads, District Nursing Leads and the Practice Development Nurse Lead. Each Lead manages a neighbourhood team, working alongside each other in a hub model to compliment and support both the therapy and nursing scope of professional practice and leadership. With a specialist qualification in Physiotherapy, the post holder will also have a wider role across CICH to support with specialist clinical supervision and lead on projects where physiotherapy specialist practice input or professional leadership is specifically required. This will include the development and strategic oversight of the Goodall Division community falls and staying steady service. The role will focus on clinical and operational leadership, professional leadership of Physiotherapists in CICH, Collaborative and partnership working, facilitating and teaching, advanced communication and research and audit.
The invaluable contribution of Occupational and Physiotherapy has been recognised in Camden community and our developing Integrated Neighbourhood model of working is integral to the successful delivery of the service. This means that the post holder will need to apply enhanced knowledge of falls management and be confident when representing the service at multiple different forums/workshops and project meetings. There is a strong emphasis on new ways of working with the Aging Well agenda, alongside multi-disciplinary neighbourhood working. The post holder will therefore also be required to act as clinical lead as a Physiotherapist to enhance multi-agency collaborative working in the neighbourhood.
1: Clinical and Operational Leadership
1.1 Ensure that the team has adequate nursing and therapy staff on duty with the appropriate skills to safely meet the needs of patients.
1.2 .To identify priorities by effective time management, ensuring the team governance Priorities are met, performance indicators, safety, risk, incident management, care delivery is managed in a proactive and timely way
1.3 Manage staff capability /performance, queries / concerns / complaints from service users, partner agencies and other team members, ensuring that feedback to others is positive and meaningful and that strengths are clearly acknowledged.
1.4 Be an exemplary role model working within the core Trust values and applying SCARF principles in all interactions with people
1.5 To display visible management and leadership skills to the integrated community team.
1.6 When required to undertake direct clinical work, working with complex cases, acting as a role model for excellence in care provision.
1.7 The post holder will act as a role model in expert practice facilitating and supporting the delivery of effective rehabilitation programmes by use of advanced clinical judgement and decision-making skills.
1.8 Ensure quality standards in holistic service provision including health promotion and education in response to the changing health needs of the local community.
10.9 Overseeing and supporting staff to develop robust risk management plans and care planning that support individual’s independence and daily living within their home setting.
10.10 Secure appropriate expertise to provide specialist support to ensure that patients are referred to other practitioners when required and that appropriate supplies and equipment are in place and reviewed as per local policy.
10:11 Work within the organisation’s framework of informed consent
10:12 Ensure that professional competency frameworks for therapy staff and students is robust and supportive to develop specialist interventions within the integrated teams.
10:13 To be conversant and have a range of in-depth knowledge of the CNWL Trusts policies and procedures ,applying them fairly in leadership decision making. . 10:14 Manage the budget and allocation of resources appropriately.
10.15 To lead on quality improvement for service efficiencies in community rehab pathways and falls group pathways.
2: Therapy Professional Leadership
2.1 To attend and represent CICH at centralised meetings and oversee specialist Occupational and Physiotherapy sign off approval for equipment.
2.2 To support and lead on all Physiotherapy and Falls clinical development work across the CRTs
2.3 To provide leadership and support with all therapy staff recruitment across CICH to recruit and retain a skilled workforce.
2.4 To support the induction, training, appraisals and probationary reviews of all new therapy staff.
2.5 With expertise in Physiotherapy the post holder will provide clinical supervision for the
Therapy team Leaders across the neighbourhood teams.
2.6 Be a professional point of contact for physiotherapy support and advice for other community teams.
2: Collaborative and Partnership Working
2.1 The post holder will be a champion of integrated working to ensure joined up seamless patient
care in the community through complex partnership working across multiagency teams.
2.2 To attend any therapy leaders’ forums and disseminate CNWL , national,
regional and integrated community service level information back to the teams.
2.3 To represent the role of the Physiotherapist at neighbourhood hubs as part of specialist multi-agency working
2.4 To facilitate and participate in Trust-wide groups and partner agency forums as required to establish, develop and maintain collaborative and productive integrated partnerships with individuals and agencies working in the field of the community health care.
2.5 Be responsible in delivering strategic improvement in the care provided to patients based on up to date evidence-based policy and practise.
3: Facilitating and Teaching to enhance quality practice
3.1 The post holder will provide specialist education and training to staff within the team to improve clinical performance.
3.2 Provide specialist therapy teaching and supervision to students and new staff as required.
3.3 Ensure the requirements and standards for student placements are being met and that junior staff have educator training.
3.4 Support the organisation of the therapy Camden rotations
4: Communication
4.1 The individual will demonstrate a high standard of electronic record keeping
and documentation with adherence to CNWL NHS Foundation Trust’s policy.
4.2 The post holder will have an in depth understanding of professional guidelines and the application of confidentiality
4.3 The post holder will work at the interface between the team and statutory and
voluntary organisations, to facilitate cohesive approaches to meeting the diverse needs of people in Camden.
4.5 The post holder will maintain efficient and effective communication both, verbal and written with clients, their relatives, colleagues and external stakeholders.
4.6 To comply with current data collation and protection systems, provide written reports as requested and complete statistical returns in accordance with CNWL policy requirements.
5: Research and Audit
5.1 To be conversant with key national policy on long term conditions, admission avoidance, adult safeguarding , National Service Frameworks, National Institute of Clinical Excellence (NICE),the model for Ageing Well and other key national strategies.
5.2 To promote evidence-based practice that responds to local and national service standards and to act as a resource to colleagues.
