Description:

The Early Intervention Team (EIT) is a very proactive and innovative multidisciplinary team of Frailty Specialist Nurses, Discharge Planning Specialist Nurses, Occupational Therapists and Physiotherapists. The Early Intervention Team has been very well acknowledged for providing proactive Admission Avoidance service, including Frailty screening, initiating comprehensive geriatric assessments for patients admitted to the Emergency Department, urgent treatment Centre and Medical Assessment Units. The service provides an excellent opportunity to develop quality improvement and service development skills as part of the service expansion.
