Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR) is looking for Registered Physiotherapists to provide services to our pediatric/young adult clients residing in group homes within the community. The position can be a flexible one, with working hours ranging from part-time to full time depending on your desired client caseload size.
REQUIREMENTS:
- Registered Physiotherapist with the College of Physiotherapists of Ontario (required)
- Experience working within a community setting is an asset but new graduates are welcome
- Ability to work independently and as part of an inter-professional team
- Ability to prioritize, manage self-scheduling and meet reporting deadlines
- Excellent oral, written and report writing skills
- Proficiency with computers
- Strong interpersonal and organizational skills
- Reliable transportation is required for this position

Responsibilities:
- Provide ministry funded, community based Physiotherapy services within the pediatric/young adults client’s place of residence and/or through virtual services
- Provide physiotherapy assessment (through interview, standardized and informal assessment techniques) in the areas of gross motor skills, co-ordination, neurological, developmental, mobility aids, etc.
- Develop and implement goal-oriented treatment plans in collaboration with clients/caregivers
- Effectively communicate information by providing education to client/caregivers and linking to community agencies/resources when appropriate
- Complete and maintain current and accurate documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and College of Physiotherapists of Ontario standards
