Description:

Job Title: Practice Nurse Location: Handforth and Cheadle Hulme Reports to: Practice Manager / Operations Manager Job Summary: The Practice Nurse plays a vital role within our general practice team, delivering high-quality nursing care and services to patients. This role involves managing and delivering a variety of clinical and administrative tasks, from chronic disease management and vaccinations to health assessments and patient education. The Practice Nurse will work collaboratively with GPs, other practice nurses, and the wider healthcare team to promote preventive healthcare and ensure optimal patient outcomes. Key Responsibilities: 1.
Chronic Disease Management: Conduct assessments, monitor, and manage chronic conditions such as asthma, diabetes, COPD, and hypertension in line with Quality and Outcomes Framework (QOF) guidelines. Develop, implement, and review individualised care plans, adjusting treatment as needed. Provide education and support to patients and their families to manage and understand their conditions. 2.
Health Promotion and Preventive Care: Offer smoking cessation advice, weight management, and lifestyle counseling to improve overall health and well-being. Provide support and information on long-term health planning, including regular screening and preventive care. 3. Vaccinations and Immunisations: Administer routine vaccinations, including flu, COVID-19, childhood, and travel immunizations, in line with NHS and practice guidelines.
Maintain accurate records of vaccinations, ensuring compliance with patient safety protocols. 4. Womens Health Services: Conduct cervical cytology (smear tests) and provide guidance on contraceptive options, in collaboration with GPs. Support family planning and offer pre-conception and sexual health advice.
5. Health Screenings and Assessments: Carry out NHS health checks, blood pressure checks, and other health screenings. Record patient observations and update patient records accurately and promptly. 7.
Patient Education and Counseling: Educate patients on managing acute or chronic conditions, reinforcing care plans, medication adherence, and lifestyle adjustments. Provide support to patients navigating their health journey, offering practical advice and resources. 8. Infection Control: Ensure infection control practices are upheld at all times, maintaining a safe environment for patients and staff.
Participate in regular audits and follow clinical guidelines to prevent and manage infections. 9. Administrative and Team Responsibilities: Maintain accurate, timely patient records and documentation in the clinical system. Contribute to team meetings, case discussions, and quality improvement initiatives within the practice.
Collaborate with GPs, nurse associates, assistant practitioners, and other healthcare professionals to provide coordinated patient care. 10. Professional Development: Engage in continuous professional development (CPD) to keep clinical skills up-to-date and aligned with best practices. Attend relevant training, workshops, and mandatory courses, maintaining an active nursing registration.
Essential Skills and Qualifications: Registered Nurse (RN)with active NMC registration. Proven experience in a primary care, community health, or similar healthcare setting. Competency in chronic disease management, immunizations, and wound care. Knowledge of QOF and NHS protocols relevant to general practice.
Excellent communication and interpersonal skills, with a commitment to compassionate, patient-centered care. Desirable Attributes: Experience in cervical cytology, family planning, and travel health. Certification in smoking cessation counseling or other preventive health practices. Strong organisational and multitasking abilities, with a proactive approach to learning and development.
