Description:
PRACTICE NURSE Accountable to the Doctors. Hours: Negotiable but must include extended hours currently on Tuesday evenings until 20:00 Job Summary: Working in accordance with the UKCC Code of Conduct and with reference to Practice Guidelines, appropriately referring to GPs, the post-holder will provide assessment, treatment, screening, health education services and advice to patients. The post-holder will work with reference to Practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners. The post-holder will have the required level of training and competence and will work within patient group directions where these are available.
A nurse with prescribing qualifications will use them appropriately. Basic Skills, Duties and Responsibilities Needed: You require at least the following: Immunisation (Adult and Child) Administering appropriate immunisation autonomously Working with patients with more complex immunisation issues. Travel Health Administering injections and providing guidance in accordance with guidelines Identifying problems Provision of specialist and evidence-based support to the team. Blood pressure recording Be able to take blood pressure and record the results accurately Urinanalysis Be able to test samples and/or pass on samples for hospital checking, as required ECG Be able to record an ECG and load it onto computer for assessment by the doctors Assessment and Advice Give dietetic advice and information Give information about Asthma re: PEFR and devices.
Computer Skills Be able to use a computer (EMIS Web), Docman 10, as necessary (though training can be given). Maintain accurate documentation of care and utilisation of I.T. Room Care Maintain the treatment/nurses room and all instruments at a very high standard of cleanliness for the purposes set out above. Stock Monitoring Assist the HCA in monitoring stock of Sample containers (blood, urine, faeces) Vaccines Syringes and needles Swabs and dressings Couch rolls and other nursing supplies Drugs stock for GPs Skills and Abilities preferred: Checks New patient Health Checks Pill Checks HRT Checks Management of Chronic Disease Identifying significant abnormalities Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals Therapeutic Monitoring Checking compliance with and adherence to appropriate treatments using an holistic patient-centred approach Recognising abnormalities Identifying the impact of treatment and implementing or recommending changes as appropriate.
Wound Care and Management Assessing routine wounds including trauma Assessing [and prescribing] to support good wound management in complex situations (e.g. use of Doppler technique) Patient Health Checks Identifying significant abnormalities Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues Working with patients to develop a management plan where health problems or potential health problems are identified Risk Assessment Recognising issues and gathering sufficient information to refer (e.g. drugs; domesticviolence; child protection; vulnerable adults; senior patients; social problems) Health Screening Recognising issues and gathering sufficient information to refer (e.g. womens and mens health, sexual health, older people) Undertaking monitoring tasks (e.g.
smears) and providing advice as appropriate Providing in depth monitoring and advice as appropriate to the level of the post-holders specialty and working with the patient in deciding on management plans. Working independently or alongside the general practitioner and making decisions/ recommendations as appropriate. Smoking Cessation Be able to advise, refer and manage patients wishing to stop smoking. Submit claims for smoking cessation work done.
DES/LES/LCS etc Be willing to act as lead on relevant commissioned and enhanced services, sending out invitation letters, and submitting claims, as may be needed. Other Dealing with more complex problems and prescribing if appropriate. Meetings/In-house Training Sessions: You are required to attend: Nurse/HCA Team meetings held every other Monday at 1.30 pm Clinical meetings, held every Tuesday at 1.30 PM Staff meetings, which are held on the second Thursday of each month from 1.30 p.m. Monthly training sessions, which are held on the fourth Thursday of each month from 1.30 p.m.
till 3.00 p.m. approximately (if applicable). Over-time: Over-time (which will be remunerated at the usual hourly rate) may be worked, as and when required, with prior agreement by a partner or the practice manager. Study Leave: You are allowed five days study leave pro rata each year (April to March) and agreement for you to attend is at the discretion of the Partners/Practice Manager.
Any additional study leave requested will be granted at the Partners discretion. Annual Leave: The post attracts 25 days’ annual leave plus Bank and Statutory Holidays per annum, April to March each year.
