2. JOB SUMMARY

The post holder will:
Play a central role in the provision of nursing services at the practice, supporting other SPC practices as required.
Ensure a high standard of professional nursing care for patients of the practice, both via personal practice and practice-wide service development.
Remain up to date with best practice and national/local guidance.
Work closely with the overall practice team to ensure best care and patient safety.
Provide teaching, training and personal development to other nurses in the practice

Responsibilities:

In accordance with the agreed practice timetable, the post holder will undertake a variety of duties including:
Run clinics for our frailty patients, including housebound, care home/nursing home patients, working with a high degree of autonomy but under the clinical supervision of the relevant lead GP(s).
Provide a range of treatment room services, in line with scope of practice, as part of the nursing team.
Provide telephone consultations to patients, where required.
Ensure clear and accurate record keeping, using relevant templates and standardised coding.
Provide health promotion and advice, signposting patients to appropriate services to maximise their health and social functioning and their ability to self-manage their care.
Be constantly mindful of the need for the provision of both personalised care and population health, including reducing health inequalities and ensure that these are reflected in daily practice.
Make professional, autonomous decisions in relation to presenting patients, whether self-referred or referred from other health care workers within the organisation or external organisations, within agreed scope of practice.
Assess the health needs of patients, taking appropriate action as necessary, including referral to colleagues inside and beyond the practice.
Screen patients for disease risk factors and early signs of illness taking appropriate action as necessary.
In consultation with patients and in line with current disease management protocols and care pathways, develop care plans for health related illnesses and share these across the multi-disciplinary team, in line with patient need and good information governance.
Work in accordance with all relevant clinical guidelines, pathways and protocols, including agreed standard data entry processes.
Support and participate in the practices work in local care homes, ensuring best care for residents who are the practices responsibility and supporting care home staff to provide best quality care within their roles.
The post holder may have tasks or responsibilities delegated to them, appropriate to their level of competence. They may also be expected to delegate tasks or responsibilities to other staff, as appropriate
