JOB TITLE: Practice Nurse HOURS: Part Time, specifically Monday 08:00-20:00, Tuesday 08:00-18:30 and Thursday 08:00-12:30. PLACE OF WORK: Based at both The Deeping Practice and Glinton Surgery RESPONSIBLE TO: Senior Practice Nurse, Practice Manager About us: The Deepings Practice is a large practice based in Market Deeping with a branch site at Glinton, we serve over 24,000 patients. The Deepings practice has a large team of clinical and non-clinical staff lead by seven partners and a full senior management team. We have a CQC score of Good and strive for excellence in all areas of patient care.
We have a strong focus on health promotion and personalised care, supporting patients to make informed decisions about their health and social care. We are a dynamic, friendly, and supportive team with experience in training GPs and other healthcare professionals. Job Summary: Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients. The post-holder will work with reference to Practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
The post-holder will have the required level of training and competence and will work within patient group directions where these are appropriate. Main Duties: Clinical Responsibilities: Management of Chronic Disease Identifying significant abnormalities within Health Promotion Clinics for chronic disease. Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals Therapeutic Monitoring Checking compliance with and adherence to appropriate treatments using a holistic patient-centred approach. Recognising abnormalities.
Identifying the impact of treatment and implementing or recommending changes as appropriate. Wound Care and Management Assessing routine wounds including trauma and post operative wounds. To support good wound management in complex situations (e.g. use of Doppler technique).
Patient Health Checks / Health Screening Recognise issues and gather sufficient information to refer where appropriate. This will. include women’s and men’s health, sexual health and vulnerable adults. Undertake routine screening (i.e.) smears.
Provide any monitoring felt to be required after assessment. Obtain appropriate information using lifestyle questioning approach and identify health promotion issues. Work with patients to develop appropriate management plans for health problems or issues that are identified. Risk Assessment Recognising issues and gathering sufficient information to refer (e.g.
drugs; domestic violence; child protection; vulnerable adults and social problems). Travel Health Providing travel risk assessments/management by administering vaccinations in accordance with guidelines/PSDs/PGDs. Provision of specialist and evidence based advice to patients. Immunisation (Adult and Child) Administering appropriate immunisation autonomously with the PGD instructions.
Working with patients with more complex immunisation issues. Mental Health Communicating with key workers. Administering appropriate prescribed therapies and monitoring for side effects. Housekeeping Cold chain maintenance and monitoring in accordance with practice policy.
Monitoring and organisation of stock levels and equipment. This job description is not exhaustive. We anticipate the role will develop with the suitable candidate. Communicate effectively with patients and carers. Recognise peoples needs for alternative methods of communication and respond accordingly. Contribution to the Implementation of Services: The post-holder will: Apply Practice policies, standards and guidance.
Discuss with other members of the team how the policies, standards and guidelines will affect own work. Participate in audit where appropriate
