To use clinical and pharmaceutical knowledge and professional judgement to improve the pharmaceutical care of patients through a range of activities which will include: complex polypharmacy reviews, long term condition management clinics, medicines reconciliation following hospital discharge, dealing with acute prescription requests, home visit medication reviews and advising GPs and other health and social care professionals on safe and effective use of medicines as appropriate. To take the responsibility of being an independent prescriber and utilise prescribing skills to improve patient care via a range of activities including complex medication reviews, common clinical condition clinics and long term condition management clinics. Be responsible for development of guidelines and protocols, and subsequent running of long term condition management clinics and for appropriate patient assessment as required To provide specialist pharmaceutical advice on a day to day basis on all aspects of prescribing and prescribing management to GPs, nurses, other practice staff and other health and social care professionals as appropriate. To review prescribing patterns and discuss recommendations with prescribers.
