Description:
Communications Lead by example to ensure timely and effective communication with patients, staff, relatives, and carers in relation to care issues and act as an advocate to ensure they have access to appropriate advice and support Ensure the confidentiality of patients and staff is maintained In a timely manner, ensure staff proactively manage patient, relative and carer concerns Seek support and advice on matters outside your experience and authority To risk assess and ensure incidents and near misses are reported as per the trusts incident reporting systems (DATIX) Liaise with the multi-disciplinary team in relation to patients care and needs Promote good working relationships within your team by exercising effective leadership skills Provide feedback to the department from courses undertaken Responsibility for Patient Care Assist the Manager in the assessment of care needs and the development implementation and evaluation of care without direct supervision Ensure all documentation is appropriate, accountable, and defensible as per Trust Policy Ensure that the nursing input within the Pre-Assessment unit contributes to care quality indicators Support the Manager to foster a culture which encourages staff to challenge and report sub-standard practice Promote the use of risk assessments and health and safety management, utilising the Trusts incident reporting system to improve the quality of patient care Assist the Manager to encourage and promote an evidence-based culture in staff using procedures and protocols of care within an inter-disciplinary approach Contribute to and support clinical effectiveness and innovative practice within the provision of care Work with the Manager to ensure that Safeguarding policies for Children and Adults at risk are adhered to by staff Assist the Manager in supporting and contributing to agreed developments within the Division Using professional judgement is able to analyse a range of patient conditions and act appropriately Maintaining high standards of patient care by personal example and professional conduct Maintain accurate and timely patients records and documentation ensuring confidentiality Planning and organising Develop clinical/nonclinical skills which will enhance patient care delivery by maximising opportunities via the Trusts Staff Development Programme Assist the Manager in ensuring that the weekly staff rota maximises the safety and efficiency of available resources Ensure timely discharge planning by working and liaising with the Multi-Disciplinary Team and Social Services Be responsible for prioritising, organising and planning own day to day workload or activities to meet the demands of the job role To regularly participate in creating staff rosters and allocation of staff as per service requirements Actively contribute and participate in the Trusts clinical governance agenda Responsibilities for Physical and / or Financial Resources In the absence of the Manager oversee the rostering of staff to ensure good skill mix and use of resources Support the Manager in actively contributing to Trust initiatives which enhances the efficient use of departmental resources Ensure responsible use of and security of assets within the department including own use and correct use by others e.g., IT equipment Ensure safe and efficient use of equipment, resources, and consumables at all times. Ensure effective use of material resources/supplies within the unit in consultation with senior staff Monitor, control and store resources/supplies according to the requirements /specifications of the clinical environment Participate in ensuring appropriate usage and monitoring of departmental resources Responsibility for Policy and Service Development and Implementation Assist the Manager to ensure Trust policies/ standards and procedures are adhered to Assist the Manager in ensuring clinical indicators and patient feedback is acknowledged and actions taken and monitored to address any short falls identified. Ensure positive feedback is reported back to staff Assist in the monitoring of standards of environmental cleanliness and challenge when not achieved or maintained To be aware of, comment on and /or actively participate in changes on policies, procedures, or service developments To embrace and support line management and the department in making service improvements effective within the workplace In collaboration with the Manager, identify service developments and plan for their implementation Promote and demonstrate innovative practice and implement changes, as required Optimize opportunities to maintain and improve professional knowledge and competence Using evidence based practice, keep ones self up to date on new trends and developments and utilize this knowledge within the unit through negotiation with the unit manager Responsibilities for Human Resources and Leadership To assist in the completion of staff appraisals Support the Manager in the recruitment and selection of staff, to ensure they have the competency, attitude and skills to match the requirements of the post Support the Manager with the implementation of human resource policies Support new or less experienced work colleagues to adapt to the workplace Assist the Manager with the planning, organisation and supervision of staff according to competencies and workload To assess clinical competencies and provide training/education and support as necessary to junior staff Raising awareness of and promoting implementation of Trust and Department Policies Participate in induction of new staff including bank and agency staff Act as mentor/preceptor to new staff Maintain continued professional development Being responsible / accountable to their own clinical / professional development including the review process Responsibilities for Teaching and Training Assist in the induction of new ward staff Participate in training and development activities that are relevant to the job role Ensure that the environment is conducive to the education and learning of all staff and students Undertake the role of mentor/ preceptor/ supervisor to other members of the ward team Assist members of the team in identifying their own training and development needs and support them in achieving individual goals Deliver health education to patients and their families in accordance with their needs Take responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with revalidation requirements and mandatory and role specific requirements Responsibilities for data and information resources Assist in the development and maintenance of staff rotas Assist in the Manager in monitoring the performance of the department and produce reports as required Ensure the effective use of I.T. initiatives within the department To participate in the Information Management and Technology (IM&T) Strategy of the Trust and ensure hand written and electronic patient records are maintained for all aspects of patient care To regularly complete electronic Direct Attendance Record To keep up to date with developments in IT as appropriate to role Research, Development and Audit Work with the Manager to maintain and evaluate outcomes of service/care provided and take appropriate action Implement research-based ideas to ensure care development To participate in research, development and relevant audit Demonstrates ability to use current research findings to develop practice and is able to critically evaluate current practice The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.
If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13
