SUMMARY:
Under the supervision of Extended Care Services Director, the Preferred Communities ICM Case Worker’s primary responsibility is to ensure that case management services are provided in a planned, effective and timely manner to eligible clients. The caseworker is responsible to ensure that the services are appropriate to meet the needs of the clients and will contribute toward the goal of self-sufficiency for participating clients. The caseworker is responsible for ensuring documentation of casework activities as well as maintaining accurate casefiles for clients. The duties extend to other refugee support programs that provide a holistic approach towards assisting refugees in our community.
MINIMUM QUALIFICATIONS:
- Education
- Bachelor’s degree preferred but not required; or equivalent of 2 – 4 years’ experience in a social service field.
- License and Credentials
- Reliable transportation
- Valid driver license, with clean driving record
- Valid vehicle insurance
- Ability to operate an agency 12 passenger van on an as needed basis for program activities and events.
- Must be 21 years of age and adhere to the CCAOSA policy, which will not allow anyone under the age of 21 to operate an agency vehicle
MINIMUM KNOWLEDGE AND SKILLS:
- Knowledge of client files documentation and record keeping.
- Skilled in using personal computer, various software, email and internet
- A solid grasp of file documentation and record keeping.
- Must be detail oriented, organized, self-motivated, work well independently and on a team.
- Must have good written and verbal skills.
- Must have good critical thinking and problem-solving skills.
- Ability to drive and maintain reliable personal transportation.
- Able to drive a 12-passenger vehicle when necessary.

Responsibilities:
- Works with clients and staff members of other refugee programs to strategize an appropriate service plan and address needs related to obtaining safe and secure housing, clothing, food, transportation, employment, education, medical and mental health care, and legal services.
- Provides direct service support and referrals as needed and works together with other refugee programs in areas of housing, employment, job training, education, legal services, counseling, health and emergency in regard to the client/client’s service plan.
- Provides direct case management services to clients to assist towards the goal of economic and social self-sufficiency.
- Reports on activities, visits, and special client concerns to the Director/Manager and other staff during required staff meetings or as needed.
- Able to input and retrieve data from an external online Refugee Database, and Internal spreadsheets for data management to keep track of all client information and program goals.
- Must complete all required professional development assigned in a timely manner.
- Reviews and maintains accurate and up-to-date client files, including intake, assessments, filing and other required paperwork.
- Participate in trainings & webinars to better equip oneself to better serve clients and attend continuing education opportunities that will increase knowledge of this position.
- Must be sensitive to the service population’s cultural and socioeconomic characteristics.
- Adhering to safety training and protocols on a daily basis and taking precautionary measure to ensure the safety and well-being of self, others.
