Description:

The post holder is for a Care Coordinator, who will receive full training and guidance to support the coordination of care through effective administration activities based in Hoveton & Wroxham Medical Centre, within the Primary Care Network of NN4. The role is administrative covering the following areas of work: Key Responsibilities: Training will be given to meet the responsibilities which include but are not limited to the following: To work within Hoveton & Wroxham Medical Centre to support personalised care, including the administration of the integrated care of patients. Summarising patient care records within SystmOne clinical IT system. Developing competences to enable clinical system data management and reporting.
Processing correspondence and associated information management to meet clinical quality requirements. Support patients to utilise decision aids in preparation for a shared decision-making conversation. Help patients to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care. Assist patients to access self-management education courses, peer support, health coaching and other interventions that support them in their health and wellbeing, and increase their levels of knowledge, skills, and confidence in management their health.
Provide co-ordination and navigation for patients and their carers across healthcare services. Helping to ensure that patients receive a joined-up service and the appropriate support from the right person at the right time. Refer patients onwards to social prescribing link workers and health and wellbeing coaches where required and to clinical colleagues where there is a clinical need. Any other delegated duties appropriate to the post.
