PROVIDER ENGAGEMENT AND EDUCATION:

  • Visit provider practice offices regularly, advising and educating on topics such as population health strategies, practice transformation, clinical integration, incentive programs, quality care gap closure, and network policies.
  • Assist providers and office staff in understanding value-based performance initiatives and provide guidance on enhancing patient care.
  • Apply advanced knowledge to communicate key messages and solutions effectively to providers and office staff.

o Practice Transformation:

  • Collaborate with primary care providers and their teams to identify areas for process improvement, care coordination, and patient outcomes enhancement.
  • Support practice change management related to emerging trends in population health, reimbursement models, and patient satisfaction.
  • Demonstrate comprehensive mastery of population health concepts and performance scorecards.

o Provider Relationship Building:

  • Develop strong relationships with primary care providers and office staff, serving as a resource for performance optimization and education.
  • Participate in meetings and discussions to review contract performance and collaborate on improvement strategies.
  • Assist in onboarding providers to essential information about CIN and affiliated payer network

o Performance Analytics Support:

  • Review quality and utilization reports, patient and payer data with providers, providing actionable insights and connect practices to relevant support for performance improvement.
  • Work closely with the Manager to provide insights into individual provider performance and identify improvement opportunities.
  • Assist low-performing physicians through performance improvement processes in collaboration with leadership, with active monitoring and effective management of performance action plans within the value-based enterprise.

o Provider Outreach and Communication:

  • Regularly communicate key messages about value-based solutions, performance models, policies, and procedures to providers and office staff.

o Continuous Learning and Improvement:

  • Participate in continuing education to stay updated on population health trends and industry best practices.
  • Propose topics or improvements based on field interactions and identified gaps in education.

o Data Management and Reporting:

  • Maintain accurate records of provider contacts, demographics, and reporting issues using designated tools in collaboration with the Network Management team
  • Provide feedback and intel related to potential needs within the provider network.
  • Maintain operational knowledge of relevant tools and systems, assisting providers in understanding and utilizing these tools.

o Collaborative Engagement:

  • Collaborate effectively with internal teams, including MSO, quality and care integration, analytics, and network management, to ensure coordinated operations.

Qualifications
The expertise and experiences you’ll need to succeed :

KNOWLEDGE AND SKILLS REQUIRED:

  • Excellent relationship management and communication skills.
  • Basic understanding of healthcare operations within ambulatory practice/provider settings.
  • Strong organizational skills and time management.
  • Proficiency in using Microsoft applications (Word, Excel, PowerPoint).
  • Knowledge of health plan/claims-related processes and data
  • Understanding of value-based care, risk readiness, and active CIN payor contracts.
  • Ability to handle complex situations and maintain professionalism.

EDUCATION AND EXPERIENCE REQUIRED:

  • Bachelor’s Degree in healthcare, business, or related field; or 2 years of related experience.
  • Experience working with physicians or in healthcare settings is preferred.

Reporting to the Manager Primary Care Performance & Enablement, this role plays a critical part in supporting the achievement of the organization’s performance goals within our Florida statewide clinically integrated network and value-based enterprise.
