An opportunity has arisen for a Band 8a Primary & Community Development Manager to join the NHS England South West Region Primary, Community and Personalised Care Team. The post-holder will be responsible for establishing the intermediate care programme infrastructure across the South West and supporting the South West delivery of the national Intermediate Care Framework.

The post holder will be part of the Primary and Community Development team, supporting the delivery of our strategic priorities to reduce health inequalities through transformed services in the primary and community adopting the key principles of personalised care.
National priorities for primary, community and personalisation focus on:

- Supporting people manage their health better and reduce demand
The post will manage delivery of defined strategic objectives arising from the programme(s).
This programme operates across the region as a Teams of teams matrix to ensure that the programme is considering the complexity of issues and opportunities to maximise the benefits to include;
- Medical
- Digital
- Analytics
- Communications and engagement
- Urgent and Emergency Care
- Health inequalities
Given this complexity of implementation of primary, community development and personalisation the post holder will support the Senior manager on specific elements of implementation and delivery.
The post holder will have regular contact with colleagues across the region and within the systems and will be expected to share learning, identify risks, issues and themes accordingly to support programme delivery. As well as deputising for the programme manager accordingly.

As a Primary and Community Development Manager, the post holder will work as part of a dynamic team in supporting systems with transformational programme implementation work as part of a matrix team across the South West region.
The current priorities for the role are focused on the needs of the population and determined with the Senior programme manager according to plan.
THE POST HOLDER WILL:
- Be a member a Teams of teams regional approach Ensure effective running of relevant boards, committee, steering group and collaborative, co-ordinating the production of highlight reports including narrative and data updates
- Be able to represent the programme at appropriate forums providing clear, concise articulation of progress, risk, issues and next steps
- Support the planning and implementation of assigned strategic objectives and monitor progress against plan for each of the systems.
- To provide management support to systems on specific areas of development
- Support the regional evaluation process to ensure that this is connected with other regions to gain and share knowledge
- Manage all necessary business returns, including programme highlight reports, weekly reports and other paperwork as required
This role will also support developing emerging priorities of work within the Primary, Community and Personalisation programme(s).
The role will champion innovation, promote quality improvement, evaluation and maximise the use of technology to better support patient outcomes through service redesign.
The Postholder will promote best practice and enable shared learning across the region and nationally.
