Description:
POSITION SUMMARY
The Enhanced Care Management (ECM) Program Coordinator is an administrator role dedicated to upkeep, management, and oversight of front-end and back-end programmatic processes. These include elements of quality assurance, compliance tracking, and team coordination.
This role operates in a fast-paced environment wherein multiple tasks are balanced concurrently. This role requires a high-level of organizational, technological, and communicational skills. This individual will also be centrally involved in communicating information
to management and will be relied on for insight into program happenings. The Program Coordinator will act as a liaison between the Street Medicine team and the Cal-AIM programs. Services will be provided to individuals that come from diverse cultural backgrounds, ensuring
that underserved communities receive the proper resources. This is a grant-funded position for 18 months.
A. EDUCATION
- Bachelor’s degree in Social Work, Public Health, Human Services, or related degree required.
B. QUALIFICATIONS/EXPERIENCE
- One (1) year of experience working in a multi-cultural community-based setting, with basic computer skills (Microsoft Office-Outlook, Excel, Word, Teams, Email).
- Comprehensive knowledge of Cal-Aim Programs preferred.
- Good communication skills and willingness to provide a positive patient experience to patients and/or their families.
C. SPECIAL SKILLS/KNOWLEDGE
- Written and verbal fluency in English required; Bilingual Spanish preferred.itive patient experience to patients and/or their families.

Responsibilities:
- Responsible for performing clerical tasks for the Enhanced Care Management and Street Medicine teams.
- Responsible for providing clear and consistent communication to Staff regarding program meetings, community resources and reporting needs per payor.
- Works collaboratively with other personnel as assigned by the leadership team.
- Reviews and assesses Cal Aim referrals for ECM and Community Supports-Housing Navigation & Tenancy Services.
- Utilize Microsoft Excel to create necessary data tracking information.
- Collect data for Cal-Aim programs, program utilization, and operation analytics.
- Provide reports to leadership team and payers as requested.
- Policy and Procedures development.
- Manage a caseload of EC
