Community Outreach Team (CREST), serves as a community safety network for people with or at-risk of mental health conditions, including dementia, and their caregivers. CREST conducts outreach events to increase mental health awareness and targeted case finding to identify those with mental health needs.
CREST works with clients aged 18 years and above.
Responsibilities:
Reporting to the Group Supervisor, the Programme Coordinator/Social Work Associate works closely with the team to serve clients and caregivers.
- Assist to plan, coordinate and execute outreach activities to educate the public, seniors and partner stakeholders on dementia and mental health awareness.
- Assessment and review information and referral service and monitoring of clients with mental health needs, and dementia.
- Networking with community partners.
- Administrative duties
- Other work projects assigned by supervisor/management
