Description:

To take a lead role in the coordination and delivery of care reviews for service users who have care commissioned by Wirral Department of Adult Social Services and/or Wirral ICB.
Lead in reviewing and monitoring commissioned services for service users, ensuring this is undertaken in a timely and person-centred way.
In conjunction with Care Coordinators or Lead Professionals ensure packages of support are the least restrictive and as local as possible whilst ensuring effective risk management and contingency plans are in situ.
On behalf of CWP work collaboratively with Wirral Department of Adult Social Services and/or Wirral ICB and providers, ensuring commissioners are aware of status of individuals who are in the category above.
Coordinate and track outcomes of reviews and ensure that Care Coordinators or Lead Professionals adhere to agreed pathways of notification of change in care package to Wirral Department of Adult Social Services and/or Wirral ICB.

An exciting opportunity has arisen to join our All Age Disabilities Care Group in Wirral as Programme Lead – Adult Social Care (Mental Health). Cheshire & Wirral Partnership NHS Foundation Trust is proud to be an integrated provider and recognises the need to ensure a strong voice for social work and social care within the organisation.
You will be a key member of the All-Age Disabilities and Mental Health Service, and will be expected to work autonomously, managing the review process for service users who have care commissioned by Wirral Department of Adult Social Services and/or Wirral ICB where a high level of clinical expertise is required. This will also involve collaborative working in the commissioning, planning and coordination of individualised care packages.
You will be required to work closely with colleagues within the Local Authority and Wirral ICB. Advanced communication skills and demonstrable experience in a practice role, delivering Mental Health (Adult Social Care) services are essential criterion for this role.
