Description:

Monitor Cancer Patient Target List (PTL) on a daily basis to ensure that patients are not exceeding the cancer waiting times standards.Actively track all Target requests referred to the Radiology department ensuring all target orders are vetted, booked and reported in the required timeframes.Ensuring patients are booked according to the appropriate target pathway; escalating issues before breeching of any target occurs and highlighting any potential delays which may affect timely patient treatment.To be proactive in the pathway management of all suspected and diagnosed cancer patients from referral to treatment.Flag any routine scans that have been escalated from routine to target with the relevant modality and ensure any appointments are vetted/booked within the Cancer Pathway waiting times.To be the first point of contact for speciality teams to answer any questions they have regarding diagnostic imaging tests for patients on Cancer pathways.Escalating to Services when next steps in pathways may delay diagnosis or treatment.To collect and record cancer waiting time and clinical data and report potential or actual breaches.Monitoring and recording the outcomes at each stage in the patients journey with particular reference to pathway tracking and collection of national cancer datasets.Track and investigate patients along their care pathway to ensure achievement of cancer waiting times targets.Use knowledge of NHS performance targets to proactively inform the Cancer Data Manager of prospective breaches and assist in monitoring and accurately reporting breeches of cancer waiting times targets.To communicate effectively with CW/WM staff at all levels and liaise with offsite staff and other tertiary health providers to ensure the required data is collected and reported according to national and local deadlines.Attend and participate in meetings relevant to the role as required.Process confidential patient-sensitive documentation to and from patients, other healthcare professionals and external agencies.Deal with concerns/complaints as appropriate, immediately escalating where necessary to the appropriate manager for prompt action.Phone patients to make appointments provide instructions, reminders and information where necessary.Ensure all patient demographics and contact details are kept up to date on Cerner
