    Your Opportunity
    The Psychosocial Support Program (PSP) is a vital program in the community in partnership with the Healthy North Coast PHN supporting people living with mental illness.
    As the Recovery Coordinator you will provide support to clients with severe and persistent mental health issues and assist them to navigate and coordinate their recovery. Help to find and set goals and to access the required supports, services, and natural supports. You will be required to undertake assessments, provide interventions, coordinate referrals, and assist clients with NDIS evidence gathering and application processes.

Your key responsibilities will be to:

  • Respond to referrals to the service from internal and external support services and conduct over the phone and formal face-to-face assessments of suitability for support
  • Induct clients into the service including the program requirements, their rights and responsibilities
  • Promote clients’ independence including developing capacity to self-manage and access required supports independently.
  • Facilitate individual and small group capacity building activities to improve client interpersonal connections to others in social settings
  • Provide intensive ongoing case facilitation (formal and informal) with clients and review progression against case plans
  • Develop strong relationships with key external stakeholders including other service providers, community service workers, government agencies etc. to assist in the receipt of information and referral of clients
  • Contribute to the completion of all required internal and external reports and assessments relating to clients and the program including risk assessments, statistics etc.

Requirements for Success

With a significant background Mental Health, you will be confident in your case planning and assessment processes. Other requirements include:

  • University Degree holder or a relevant Associate Diploma or certificate with relevant experience or other appropriate qualification/experience acceptable to Mission Australia.
  • Alternatively possesses experience attained through previous experience at Mission Australia and/or study of an equivalent level of expertise.
  • Understanding of the social determinants of health, trauma informed care, as well as lived experience and personal recovery frameworks.
  • Ability to work in a strengths-based framework to facilitate client choice, safety and responsibility.
  • Demonstrated professional approach to the role including maintaining professional boundaries.
  • Demonstrated ability to build and maintain sustainable internal and external relationships.
  • Demonstrated ability to deliver group work under the direction of a senior employee.
  • A positive and person-centered and recovery orientated approach.
  • Sound written and verbal communication skills, including proficient administrative skills and use of technology (database, mobile devices).
  • Personal development and self-awareness including mastery with own wellness and confidence in own resilience.
  • Must have Full driver license with no restrictions
