Gellert Health has transformed healthcare for the most vulnerable and underserved populations by providing Health Navigation Services aimed at enhancing both physical and behavioral health outcomes for patients. JOB SUMMARY:

The Referrals Program Coordinator at Gellert Health plays a key role in supporting our mission to deliver exceptional Health Navigation services with kindness, integrity, and quality. Reporting to the Director of Navigation, this is a frontline position that focuses on acquiring new patients, ensuring a smooth referral process, and providing a positive onboarding experience.
The role involves maintaining strong relationships with referral partners, insurance companies, and other stakeholders, while effectively communicating our mission, services, and criteria via phone, virtual platforms, and in-service presentations.
Ideal candidates are proactive, enthusiastic, and solution-oriented, with excellent client service and communication skills. Experience in sales and marketing or patient acquisition role in the healthcare-related field is preferred. A passion for building positive, collaborative relationships is essential.

2. RELATIONSHIP MANAGEMENT AND EDUCATION OF CURRENT REFERRAL PARTNERS & INSURANCE COMPANIES

  • Develop and maintain strong, collaborative relationships with existing partners and insurance companies.
  • Work closely with Referral Partners to understand their needs and improve service quality and delivery.
  • Assist with onboarding of new referral partners, ensuring their understanding of Gellert Health’s services, criteria and processes.
  • Provide education, training and communication virtually or in-person to Referral Partners to enhance and increase the quality and quantity of good-fit referrals.
  • Develop and share marketing materials that highlight Gellert Health’s navigation services and patient outcomes.
  • Address and resolve any obstacles or challenges faced by Referral Partners in the referral process.

NECESSARY SKILLS & QUALIFICATIONS:

  • Strong professional communication skills, both verbal and written, and excellent phone handling skills.
  • Proficient with public speaking to present and promote our services and build strong relationships.
  • Willing and able to drive periodically to attend meetings at partner locations or to attend relevant business events and conferences as needed.
  • Demonstrates ownership and accountability for results independently and with the team to meet monthly referrals goals.
  • Empathy, compassion, and resourcefulness in addressing patient and partner needs.
  • Highly organized in managing and prioritizing tasks in a dynamic, fast-paced environment.
  • Resilient problem-solving skills with a focus on customer service and collaborative teamwork.
  • Proficiency in using electronic medical records and basic computer systems.
  • Ability to maintain confidentiality and comply with HIPAA guidelines.
  • Excellent relationship-building skills with internal teams, referral partners and patients.
  • Experience with physical and behavioral health services.
  • Experience working with culturally diverse and vulnerable populations is preferred.

REQUIREMENTS:

  • Bachelor’s degree
  • Minimum 3 years of relevant work experience, e.g., sales and marketing, patient acquisition or relationship management in the healthcare or related field.
  • A valid Arizona driver’s license with a clean driving record.
  • Ability to pass background check and drug testing.
  • Active AZ Fingerprint Clearance Card
  • Flu Shot (waiver available with possible mask requirement)
  • TB Test

TRAINING AND ASSESSMENT OF NAVIGATION SKILLS AND COMPETENCY:

  • Successful completion of orientation, shadowing and the Certified Peer & Recovery Support Specialist training provided by Gellert Health
  • Weekly supervision and ongoing feedback and monitoring of job performance
  • 90 Day Review & Annual Evaluation
