Description:

Join us as a Registered Dietitian (RD) in the new and innovative CBI Home Health Durham OHT Leading Practice program, where you will be at the forefront of transforming health and care delivery in our community! As part of a dynamic new team embracing a Neighbourhood Care Model, you will have the flexibility and autonomy to make a real impact on the lives of diverse individuals and communities. Collaborating closely with an interdisciplinary team, you will develop tailored care plans and interventions that empower patients to reclaim their independence and thrive in their daily lives. This is not just another job; it’s a chance to engage in a groundbreaking initiative that prioritizes person-centered care, with ongoing mentorship and training opportunities to support your professional growth. Together, we will leverage creative strategies and community resources to ensure everyone has equitable access to the care they deserve. Come help us do things better and differently!
