POSITION SUMMARY

The Dietitian is part of an interprofessional team supporting community-based patients within our communities. The focus of the role is on health promotion, disease prevention and management through individual consultation for patients ages 6 and up. The role is also a key member in our workshop program delivery as a group workshop facilitator.

Responsibilities:

Patient Care (Individual Nutrition Consultation)

  • Conduct individual nutrition assessments for patients ages 6 and up.
  • Provide short-term support through education and counseling using a patient-centered approach for a variety of health-related concerns.
  • Develop, implement, evaluate, and adjust nutrition care plans specific to patient needs and goals using an effective, safe, evidence-based approach.
  • Complete and document the Nutrition Care Process (NCP) using Nutrition Care Process Terminology (NCPT) in an electronic medical record.
  • Provide coordination of care by identifying and initiating cross-referral opportunities to programs within and outside of the SCPCN to meet patient needs, as required.
  • Communicate pertinent patient information to referring physicians and/or team members in a timely manner to support seamless patient-centered care.

Group Facilitation

Facilitate and co-facilitate in person and virtual group workshops for a variety of health-related topics for adults of all learning styles, health literacy and skill levels.

Manage conflicts or disagreements that may arise during the group workshops, as required.

Work collaboratively with interdisciplinary team members and complete community needs assessments to develop, facilitate, and evaluate group workshops.

Provide educational sessions to physicians, community members, and other health team members, as required.

General duties

  • Collaborate with the Program Manager and team and to identify and develop unique and innovative approaches to address diverse patient needs and maximize patient care outcomes.
  • Identify and pursue opportunities for continuing education to maintain anddevelop skills.
  • Support the orientation of new members and practicum students under the direction of the Team Lead.
  • Actively participate in meetings, ongoing program development, statistics, evaluation, and quality improvement opportunities.
  • Assist with implementation of new protocols.
  • Act as a resource for other staff and practicum students, as appropriate.
