Description:
The Registered Nurse provides safe therapeutic care in a holistic and systematic way, incorporating differences into the provision of care. Integrates knowledge, skills, and experiences to meet the needs of patients and families throughout the continuum to include patient and family education. Relies on education and training to complete responsibilities within scope of licensure as registered nurse.
This position will focus on population health for heart failure patients.
Responsibilities to also include:
- Assess, plan, implement, and evaluate educational activities planning for an assigned population.
- Implement new programs based on community health needs and strategic planning discussions.
- Assess patient health problems and needs according to the latest nursing standards, develop andimplement nursing care plans and maintain accurate and complete medical records.
- Coordinate necessary health-related services for patients within and outside of this facility.
- Maintain open channels of communication between all appropriate providers, patients, andadministration.
- Continually evaluate patient’s compliance with plan of care and regularly communicate updatesto provider(s).
- Provide general nursing care to clinic patients.
- Perform all other job duties as assigned.
EXPERIENCE, EDUCATION, TRAINING, SPECIAL SKILLS, AND LICENSURE:
- Education: Graduate of accredited school of nursing
- Skills: Proficient in the English language, verbal and written communication skills, computer skills
- Licensure: Current Louisiana State license as RN and BLS certification
- ACLS within 6 months of hire
