Description:
Position Summary:
The Registered Nurse Case Manager- Probation and Parole Office will:
· Provide intensive case management to adults 18 years and over with complex physical health, mental health and/or addictions needs who are offenders serving their sentence in the community, managed by Probation and Parole Officers.
· Work under the supervision of the Clinical Nurse Coordinator of the CMHA Primary Health Care Clinic and report to CMHA Hamilton’s Director of Clinical Services
· Work collaboratively with clients within a framework of self-determination to develop client-centred, relevant, recovery-oriented goals with specific activities and measurable, time-based outcomes.
· Advocate for people with a history of recidivism, homelessness, temporary or precarious housing, and challenges accessing primary care and facilitate seamless transitions to care and support.
· Facilitate equitable access to care, offering both in-person and virtual service on-site at Hamilton’s Probation and Parole office(s), in community, and at CMHA Hamilton.
Qualifications:
Candidates must possess a degree in nursing, registration with the College of Nurses of Ontario and current practice insurance. Candidates must have a valid driver’s license and a reliable vehicle for use in this position. Experience working with individuals with serious mental illness and/or substance use disorders is required. The ability to offer French-Language services is an asset.
