Description:
Coordinates and delivers patient care using the health care team philosophy practice model. Coordinates activities with ancillary departments. Community outreach workers act as a liaison between community programs and services and community members. Particularly offering specialty care to populations in rural areas. The outreach nurse position is designed to bridge the gaps in care, care coordination, and resources for seniors and their care partners during difficult aging situations. We currently assist populations surrounding Bolivar County, Warren County, Lincoln County, Attala County and soon to be Scott County Mississippi. This requires traveling to these counties 6+ times a month, with the primary working location being in Jackson, Mississippi. Maintains standards of department and the hospital.
Essential Functions
- Provides the best possible nursing care by planning, organizing, and directing the nursing functions of patients.
- Provides a positive and professional representation of the organization.
Education - Graduate of an accredited nursing program.
Experience - Previous experience in general duty nursing preferred, but not required.
Special Skills - Employee must demonstrate ability to recognize patients’ individual needs based on medical conditions, age adolescents, young adults, middle-aged and geriatric, limitations and planned procedures. Requires good judgment in delivering patient care; good oral and written communication skills; ability to function in stressful situations. May occasionally work overtime.
