POSITION DESCRIPTION
ONTraC (Ontario Transfusion Coordinators) Program is a Ministry of Health funded program that continues to support a provincial Patient Blood management program through placement of Patient Blood Management Coordinators within Ontario Hospitals.
The Registered Nurse - Patient Blood Management (PBM) Coordinator role is vital in enhancing optimal transfusion practices and providing alternatives to blood transfusion where appropriate. The hospital for Sick Children is a participant in the Ontario Transfusion Coordinators (ONTraC) program. The Registered Nurse - PBM Coordinator will work in Transfusion Medicine in the Department of Paediatric Laboratory Medicine at SickKids.
The Department of Paediatric Laboratory Medicine (DPLM) at SickKids is a leading, world-class facility working towards reducing the impact of childhood illness by providing diagnostic information that allows for early selection of best treatment options.
Patient, Family & Staff Education:
- Ensures that patient/family focused support and educational information is provided to those patients who may require a blood/blood product transfusion.
- Participates in the education of hospital staff (example, nurses, physicians, surgeons, technologists, pharmacists, etc.) and patients regarding PBM.
- Develops blood management algorithms appropriate for the hospital in accord with ONTraC policies and procedures
Responsibilities:
Management of a Patient Blood Management Program:
- Provides ongoing support for the Provincial Blood Utilization Strategy by performing a variety of activities that will contribute to establishing and maintaining a PBM Program.
- Screens and enrolls qualified patients in the appropriate PBM programs and facilitates referrals to appropriate services.
- In collaboration with the health team, develops treatment plans and administers treatments.
- Participates in development of hospital PBM policies, standard operating procedures and medical directives.
Patient, Family & Staff Education:
- Ensures that patient/family focused support and educational information is provided to those patients who may require a blood/blood product transfusion.
- Participates in the education of hospital staff (example, nurses, physicians, surgeons, technologists, pharmacists, etc.) and patients regarding PBM.
- Develops blood management algorithms appropriate for the hospital in accord with ONTraC policies and procedures.
Data Management and Analysis:
- Ensures maintenance of a current, complete, and accurate database for all patients enrolled in the Patient Blood Management Program.
- Responsible for data collection, data reporting, data accuracy, and dissemination of data.
Institutional and Regional Activities:
- Participates with ONTraC program management in developing hospital PBM benchmarks for performance evaluation.
- Attends ONTraC and other meetings and participates as a member of the PBM Committee and/or the Transfusion Committee, as well as regional PBM committees.
- Communicates with fellow PBM Coordinators, referring physicians and institutions within the region.
- Facilitates (with hospital administration) the site visits of the program Manager and Program Director
- Participates in hospital program development and implementation activities using a continuous quality improvement approach.
