Description:

Reporting to the Clinical Manager, Outpatient Mental Health and Addictions (OMHAP), the Registered Nurse (RN) provides safe, competent and effective care in situations of health, illness, and disability across the life continuum while promoting nursing excellence and high professional standards. The RN will provide care within a collaborative and inter-professional clinical team environment that reflects recovery/client-centred care principles and values. The successful applicant will provide support to the adult outpatient mental health department within central intake, CAPS and medication clinic with the primary responsibilities of: intake and screening assessments; education; system navigation; disposition support; follow up and supportive counselling; program and system development and evaluation to ensure adult clients are referred and connected to the most appropriate program and other duties as prescribed. The successful applicant will work with new referrals, and discharged adult clients, to assess mental status, risk status, respond to questions regarding the referral process and discharge plans, provide crisis support, coordinate additional referrals as required and to identify the need for further contact and intervention. The successful applicant will utilize various therapeutic modalities based on best practices for adult individuals with mental health and addictions issues that include skill building, CBT/DBT approaches, motivational interviewing, psycho-education, interpersonal effectiveness and recovery strategies in addition to providing support and education to clients and their families affected by mental illness and/or addictions through collaboration with internal and external partners.

QUALIFICATIONS:

o Registration with the College of Nurses of Ontario (CNO) in good standing required
o A minimum of three (3) years related adult clinical experience in a mental health setting working with individuals and families living with complex mental health issues required.
o Formal education in Cognitive Behavioral Therapy, Dialectical Behavioural Therapy, Motivational Therapy and/or Acceptance Commitment Therapy (more than workshop hours) required.
o Demonstrated knowledge in the use of Cognitive Behavioural Therapy, Acceptance Commitment Therapy, Motivational Interviewing and/or Dialectical Behavioural Therapy, required.
o A minimum of one (1) year previous experience in providing crisis support and management required.
o Current Basic Cardiac Life Support (BCLS) certificate required
o Current SMG (Safe Management Group) or Non-Violent Crisis Intervention training.
o Requires extensive knowledge of Niagara Health’s Outpatient Mental Health and Addiction services, community mental health and addiction resources and LAI- long-acting injectable medication and Clozapine treatment
o Requires knowledge of mental health diagnoses and psychopharmacology, particularly long-acting injectable medication and Clozapine
o Demonstrated ability to conduct adult comprehensive bio-psychosocial mental health assessments (including mental status examination, Abnormal Involuntary Movement Scale (AIMS), medication side effects, risk assessment and substance use assessments) with development of crisis plans, as needed.
o Demonstrates superior knowledge in engaging with adult clients and/or families who may be reluctant to seek help and establish rapport in various scenarios.
o Strong ability to build effective internal and external partnerships to support care planning, care transitions, and OMHAP system development.
o Demonstrated knowledge of College of Nurses of Ontario (CNO) Standards of Practice and commitment to maintaining reflective practice standards as described by the CNO.
o Demonstrated knowledge of Canadian Standards for Psychiatric-Mental Health Nursing, as well as relevant legislation including the Mental Health Act, Health Care Consent Act and Personal Health Information Protection Act (PHIPA).
o Preference will be given to candidates who have the Canadian Psychiatric Mental Health Nursing Certification (CPMHN) and/or candidates willing to obtain this certification within 3 years.
o Demonstrated commitment to professional development, ongoing education and integration of research into clinical practice.
o Demonstrated organizational, problem-solving, decision-making, conflict resolution and critical thinking skills.
o Demonstrated excellent interpersonal skills and effective oral/written communication skills.
o Ability to foster collaborative working relationships, as part of the inter-professional team, and able to work independently
o Applies NH values, professional practice standards, recovery/client-centred philosophy to patient care and service.
o Must be willing to work flexible hours to reflect patient population needs.
o Consistent and regular attendance required.
