Salary: $39.07 to $56.00 per hour (start rate in accordance with collective agreement)
Shifts: Day/Weekend/on call
Status: Temporary Full-Time (Contract end date April 2026)

POSITION SUMMARY

IMPACT is an Assertive Community Treatment (ACT) program that provides team treatment, psychosocial rehabilitation and support in the community to adults with severe and persistent mental illness and a diagnosis of a primary psychotic disorder. Its interdisciplinary team includes Psychiatrists, Psychiatric Nurses, Social Workers, Occupational Therapist, Community Mental Health Worker, Peer Support Worker, Psychiatric Resident, as well as students from different disciplines. IMPACT strives to minimize the impact of mental illness on clients’ lives, by facilitating the integration of clients into communities of their choice and supporting them to improve the quality of their lives, in the areas of health, housing, finance, and vocation. 

QUALIFICATIONS

  • Current registration with the College of Nurses of Ontario required
  • Basic Cardiac Life Support Certification required
  • BScN degree preferred
  • Two to three years current experience working with psychiatric patients, preferably community-based psychiatry
  • Demonstrated involvement with people who have severe and persistent mental illness
  • Proven knowledge of psychosocial rehabilitation/recovery model
  • Experience in substance abuse treatment and harm reduction an asset
  • Strong time management, problem solving, and organizational skills
  • Proven patient-centred care approach
  • Effective written and verbal communication skills
  • Experience working in an interdisciplinary team environment an asset
  • Proven crisis intervention skills
  • Ability to work effectively and efficiently under pressure
  • Knowledge of mental health policies, legislation, (ie. Occupational Health & Safety Act, Mental Health Act) and other relevant legislation
  • Knowledge of community resources pertinent to consumers of the mental health system
  • Computer and technology literacy
  • Driver’s license and access to a reliable vehicle is required

Responsibilities:

  • Primary case coordinator for a caseload of 10-12 clients
  • Contributes to the development of interdisciplinary treatment plans
  • Establishes & maintains a therapeutic relationship with clients, their family members, and significant others
  • Collaborates with team members regarding clients’/families’/significant others’ concerns and goals
  • Facilitates appropriate referrals based on clients’ goals
  • Provides practical assistance, advocacy, coordination, side-by-side individualized solution-focused support, and psychoeducation to help patients obtain services and skills for daily living (i.e., medical, dental, legal, housing, finance management)
  • Participates in daily interdisciplinary clinical rounds
  • Administers medications and collaborates with team psychiatrist to complete medical assessments of clients’ physical health
  • Coordinates psychiatric treatment with physical health treatment
  • Receives, validates, and secures weekly medication delivery from pharmacy
