Description:

Job purpose: a) To provide and monitor safe, person centred care to patients in a range of care settings under the direction of a Registered Nurse, without direct supervision in line with an agreed plan of care. b) To support the registered nurse in the contribution to on-going assessment, treatment and care planning for patients c) To promote health and prevent ill health in patients and service users and contribute to integrated care d) To monitor the condition and health needs of patients in partnership with colleagues, families and carers, referring to others for reassessment, when required e) To actively contribute to an effective learning environment and support others in their learning f) To adhere to the Nursing and Midwifery Code of Conduct for Nurses, Midwives and Nursing Associates and work within their scope of practice following the professional standards of practice and behaviours for nurses, midwives and nursing associates and the standards of proficiency. Provision and monitoring of assessment, treatment and care: a) Provide holistic, compassionate, safe and effective interventions, care and support to patients, their families and service users in a range of care settings under the direction of a registered Nurse, without direct supervision and in line with an agreed plan of care. b) Prepare for, carry out and monitor assessments and treatments with patients in line with competence level.
c) Develop understanding of all elements of the nursing process and be able to assist the Registered Nurse in the ongoing assessment, planning, management and evaluation of care. d) To work as a member of the multi-professional team providing clinical care to patients in all settings within the Trust, as supported by the Registered Nurse or registered care professional e) Actively engage with individuals, their families and/or carers and contribute to risk assessments and care planning, by establishing their needs, wishes, preferences and choices and incorporate these into care planning. f) In community teams plan and prioritise own visits to delegated patients, ensuring this is documented on the patients electronic health record. g) Undertake enhanced clinical interventions including but not limited to, catheterisation, venepuncture and ECGs.
Apply competency based treatment techniques/care to patients with specific conditions under the indirect supervision of a qualified practitioner, following a prescribed treatment/care plan. h) Ensure the privacy, dignity and safety of individuals is maintained at all times. i) To act as an advocate for the patients and the family, including promoting and protecting their interests, privacy, rights and inclusion. j) Understand the role of a range of professionals and carers from other organisations and settings who may be participating in the care of a person and their family.
k) Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered Nurse or other registered care professionals as appropriate. l) To ensure that high standards of nursing care are given and maintained. To act and raise concerns when standards are not being maintained following local policy and procedures. Demonstrate the proficiency to administer medications within local policy and procedure and in line with the NMC Standards of Proficienc
