Reporting to the Care Manager of Addiction and Mental Health Supported Housing, the Registered Nurse (RN)/Registered Psychiatric Nurse (RPN) will be a key member of a multidisciplinary team providing mental health support to clients with mental illness and potential comorbidities in various supported housing settings. AMH Supported Housing is an interdisciplinary, recovery-oriented program aimed at supporting individuals in achieving stability and improving their quality of life within supported housing settings. Key Responsibilities include: Mental Health Support - Provide comprehensive mental health services to clients, ensuring individualized care plans are effectively implemented; Support Client Goals - Assist clients in setting and achieving their personal goals, promoting their independence, and enhancing their quality of life; Interdisciplinary Collaboration - Work closely with other programs and agencies to build capacity within supported housing environments, promoting a recovery-oriented approach; Scope of Practice - Operate within the RN/RPN scope of practice to assess, coordinate, supervise, administer, and evaluate health services and resources; Liaison Role - Act as a liaison between clients, families, natural supports, team members, contracted operators and their staff, physicians, mental health therapists, and other relevant programs and agencies; Intake Participation - Assist in intake activities to ensure appropriate placement and support for new clients. As a Registered Psychiatric Nurse (RPN), you will provide leadership through a wide variety of nursing services to patients (clients, residents) for the purpose of promoting, maintaining or restoring health, mental health and preventing mental illness, injury and disability. You will teach, counsel and advocate on behalf of patients and take necessary steps to ensure their safety and well-being. You will provide safe, quality patient and family centered care, while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Psychiatric Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Psychiatric Nurse in accordance with AHS/UNA Article 25.02(a)(i) or (ii) as applicable.
