Description:

Home First are excited to be able to advertise for a Band 4 Rehabilitation Assistant post.
Working to support the Physiotherapy and Occupational Therapy needs of people in Milton Keynes, Home 1st provide holistic assessment and treatment plans in community and inpatient rehabilitation settings. This role will assist us to provide rehabilitation programmes for patients who have complex physical needs or require further therapeutic progression to achieve their goals and independence.

You will help prevent our patients being admitted to acute services and support their discharge from hospital. Aiming to improve their independence in functional skills, this role will require use of your empathy, communication and partnership working skills. You will have the opportunity to have supervised support from the Home 1 st therapists and work in co-operation with the wider multi-disciplinary team in providing rehabilitation needs and discharge planning to maintain patient flow. Previous experience of working in a physical rehabilitation role is essential.

Interviews will be taken place in a face to face format
To undertake specific skilled support work to aid rehabilitation of patients, to achieve optimal independence with or without the assistance of others.
To provide flexible programmes and interventions of therapeutic activities, under the guidance of Home1st Allied Health professionals (AHP)
Follow treatment plans, agreed by the AHP.
Work without direct supervision, monitoring progress and minor adaptations to interventions as required, including low level assessments.
Participate in planned therapy interventions and unplanned admission avoidance activity.

Under the guidance of an AHP, carry out assessments and interventions in the community predominantly patients own home, but this could include any Home 1 st Therapy settings.
Participate in completing competencies to develop skills in assessment and safe treatment planning.
To provide high standards of documentation and communication, including the use of electronic database i.e. System1
Carry out other duties deemed necessary by the AHP in discussion with the post holder to ensure adequate provision of service, this may include supporting Therapists in A/E department MKFTUH.
Accept allocated work form the AHP. Gather information from patients, family, carers and other healthcare and community professionals as directed by the AHP.
To re-validate patient consent, initially obtained by the AHP and be able to work within a legal framework with those patients who are unable to give informed consent.
To provide a support role in assisting the AHP in performing assessments and care planning.
To provide a primary role in assessing patients, deemed appropriate to delegate to the RA, by the AHP and provide feedback regarding the patient’s needs seeking support from the AHP to ensure safe clinical practice.
To undertake Nursing duties, as agreed with the AHP and with access to supervision from them.
To use information gained from assessment to develop a problem list and proposed intervention options. To action Therapy interventions following discussion with the AHP.
To carry out Therapy tasks which may involve moving and handling and personal care.
Support patients with arranging GP visits, prescription requests and collections.
Following appropriate training, develop competencies in Nursing and Therapeutic interventions.
To ensure the patients comfort needs are met after intervention e.g. drinks available as appropriate / provide means to contact care staff. Seek assistance from an appropriate source as required.
To prepare and restore the environment for and after intervention by self or as directed by the AHP e.g. clearing the bed space area or ensuring the intervention areas is appropriately set up for intervention.
To prepare the equipment required for intervention by self or as directed by the AHP.
To effectively use a range of verbal and non-verbal communication tools to engage and negotiate with patients across a multicultural and intellectual spectrum. This will include patients who may be elderly, frail or vulnerable, and have difficulties with understanding or communicating e.g. vision / hearing impaired, dysphasic, cognitively impaired or those who do not have English as a first language
