Description:

We are looking to recruit a motivated energetic and experienced renal nurse into the Renal Home Therapies Team supporting patients at home whilst carrying out their own dialysis treatments. The post holder will care for a cohort of patients, working mainly 8am-4pm Monday to Friday, but with flexibility to work in the evenings when required. They will also be on-call at weekends (one in six weekends) to support the Assisted PD Team.
All applicants must hold a UK driving licence and have access to a reliable car for work.
To teach patients how to carry out their own Peritoneal Dialysis treatment at home and to provide specialist nursing care and support to patients and their carers at home, maintaining and developing a quality service.
To support patients and their carers carrying out Haemodialysis at home.
To visit patients who are approaching the need for dialysis and educate them about their choices around dialysis treatments
To take overall responsibility for the care of a caseload of patients requiring specialist nursing care within the hospital and home setting.
To provide specialist training, to both staff and patients and their carers, as required.

JOB SUMMARY

To teach patients how to carry out their own Peritoneal Dialysis treatment at home and to provide specialist nursing care and support to patients and their carers at home, maintaining and developing a quality service.
To support patients and their carers carrying out Haemodialysis at home.
To visit patients who are approaching the need for dialysis and educate them about their choices around dialysis treatments
To take overall responsibility for the care of a caseload of patients requiring specialist nursing care within the hospital and home setting.
To provide specialist training, to both staff and patients and their carers, as required.
To provide an on-call service for the Assisted APD Team
The post holder will be based at: Renal Unit, City Campus but will be making regular visits to patients in their homes, so will need a full UK driving licence and access to reliable transport

Responsibilities:

Clinical
Teach patients how to carry out their own Peritoneal Dialysis treatments ; both CAPD and Arrange for the dialysis equipment to be delivered to the patients home
1.2 Support, monitor and care for a cohort of patients, and their families, carrying out peritoneal dialysis and haemodialysis treatments at home.
1.3 Work within the multi-professional team to deliver specialist clinical care both in the patient/client home, and within the Home Therapies Unit appropriate to the needs of the patient group and based on principles that are research based and which enhance the quality of patient care.
1.4 Assess and evaluate specialist clinical care using a range of assessment tools, clinical guidelines and approaches (eg. Pain, sleep, nutritional status, speciality specific tools). Take appropriate action as required.
Recognise and act on signs and symptoms of common conditions or complications within the same patient / client group.
1.6 Be a visible point of contact for patients, visitors, relatives, and staff, acting as a resource for problems and able to clearly present the patients’ / clients’ points of view to others. Receive and investigate any complaints received.
1.7 Act as the patient’s advocate ensuring that their best interests are met.
1.8 Consider and effectively deploy available resources to achieve safe care.
1.9 Always act with regard for the privacy and dignity of the patient. Cooperate with and participate in research and clinical audit undertaken in the clinical area in order to carry forward the development of research based practice.
2.0 Participate in the support and implementation of Essence of Care guidelines.
2.1 Ensure that patient care plans are regularly updated, changes implemented and that patients’ records and documentation are maintained and current.
2.2 Attend regular meetings to discuss patient management as required.
2.3 Participate in the development of evidence – based patient care policies and procedures. Develop and contribute to clinical guidelines and protocols where appropriate and ensure they are implemented.
2.4 Provide specialist support to the clinical team in dealing with the complex needs of different client groups and when imparting or receiving sensitive, complex or contentious information.
2.5 Assist with the identification and facilitation of clinical audits to promote clinical effectiveness as part of the Directorate’s Clinical Governance Framework.
2.6 Adopt innovative ways of working within the speciality.
2.7 Work at least 80% of time in clinical practice, with the emphasis on delivery of a quality service to patients both in the hospital and home setting.
2.8 Support the matron and nursing team in developing nursing within the speciality, leading and implementing innovative ways of working.
2.9 Comply with all Trust policies with particular regard to the policies and procedures carried out in the event of maintaining staff and patient safety.
3.0 Report all accidents and untoward incidents to the nurse in charge of the area and complete an incident form in accordance with Trust policy.
3.1 Report problems with equipment and facilities to the nurse in charge
3.2 Be prepared to attend and undertake any necessary training, being aware of the need to develop oneself and to strive to increase personal knowledge.
3.3 To provide on-call telephone advice to the Assisted APD Team when required
Communication
2.1 Liaise closely with other health and social care professionals to ensure a coordinated approach and high quality care is provided for patients.
Convey and present specialist information, written or verbal, formally or informally, as appropriate. Report appropriate clinical information to senior staff. Ensure that written documentation is kept in respect of all investigations and incidents.
Establish professional relationships and maintain effective communication.
Link with professional networks across the Trust and wider health community, in order to develop and exchange ideas for improving clinical care.
Convey complex and sensitive information (eg. abnormal test results) in a comprehensible and empathic manner.
Discuss all alternative treatment options and facilitate clinical management decisions.
Obtain informed consent for procedures as required, in accordance with the Trust consent policy.
Provide ongoing liaison and support for patients / clients within care pathways in the home setting.
