Title: Residential Occupancy Housing Case Manager
Department: Public Works – Residential Occupancy
Reports to: Residential Occupancy Manager
Designation: Full-Time
Review date: Three (3) Months After Start
JOB SUMMARY
The Residential Occupancy Housing Case Manager provides support to the Community Housing program. This role involves working with Generational Homes in emergency situations, assisting occupants to keep their housing by engaging with them to address issues with mental illness, substance use, physical health, and life skills. Building relationships with occupants and collaborating with them to achieve their goals.
QUALIFICATIONS AND SKILLS:
- Desired Education and Experience:
o Minimum High School Diploma or GED.
o A Diploma or Certificate in Social Work is an asset.
o Non-violent crisis intervention training an asset.
o Standard First Aid with Level C CPR Certification.
o Experience with providing support services to individuals facing challenges surrounding maintaining housing accommodations.
o Proficient in MS Applications.
- Personal Attributes:
o Ability to work effectively within a team setting.
o Knowledge of needs, experiences, and community services for the homeless population.
o Ability to work sensitively and effectively with vulnerable populations.
o Demonstrated administrative and organizational skills.
- General Employment Requirements:
o Criminal Record Check, and Vulnerable Sector Check.
o Valid Driver’s License and reliable transportation.
o Oath of Confidentiality.

Responsibilities:
- Perform home visits, call for updates, and collect information for program compliance.
- Maintain up-to-date case notes, daily log-records, progress reports and occupant files.
- Completing intakes and documenting casework meetings, goal plans, safety plans, and critical incidents.
- Provide mediation and advocacy with occupants on Public Works behalf to develop a workable plan to obtain and or maintain housing.
- Represent Tsuut’ina Nation in a professional manner and uphold the Mission of Tsuut’ina Nation.
- Ensure that Tsuut’ina Nation’s policy regarding confidentiality is always maintained, and that job responsibilities are carried out in a professional, effective, and efficient manner.
- Ensure that Housing policy and procedures, and Occupant Agreements are adhered.
- Keep accurate transitional, and emergency housing list.
- Provide information and referral assistance regarding available support from appropriate social service agencies and community programs.
- Educate clients of their rights and responsibilities under the Occupant Agreement.
- Provide compliance reporting for internal records, reports, lists, internal/external correspondence.
- Maintain a complete working file providing activity documentation and copies of all corresponding paperwork.
- Assume other responsibilities as assigned.
