Description:

Job Summary: As a respiratory specialist nurse, the post holder is responsible for ensuring the effective running of the Interstitial lung disease (ILD) and Pulmonary Hypertension (PH) service in the absence of the Senior Respiratory Nurse Specialist. This is a developmental role depending on experience. The successful applicant will commence at band 6 AFC and be expected to complete the non medical prescribing course. Once complete and fulfilling the band 7 criteria they will be elevated to that banding.
The post holder is responsible for the assessment, planning, implementation and evaluation of patient care during their span of duty. They will undertake a range of duties to ensure the effective delivery of care. With support they will undertake specialist procedures, provide advice and on-going support to the client group. They will run nurse led clinics, be involved in Multidisciplinary team meetings, provide ward and telephone support and home visits.
As a senior nurse the post holder is responsible for providing specialist knowledge, advice, support and care to patients, carers and other health professionals relating pulmonary fibrosis (PF) and Pulmonary hypertension. The post holder will contribute to the development of the service, undertaking research and audit within this speciality; developing protocols, procedures, standards and the tools to implement care. Knowledge and Skills 1. Communication and Relationship Skills Actively supports patients and their families, communicating difficult and complex information using a variety of methods, including digital technology to ensure effective understanding.
Troubleshoots concerns, and seeks early resolution to address these. Demonstrates empathy and compassion toward patients and their families when communicating sensitive information about their condition. Develops effective relationships with patients to assist them in making decisions about care their care and explaining treatment options. Ensures effective communication is maintained between members of the multi professional team during the span of duty.
Ensures information is conveyed to the patient and family in a timely and understandable manner. Establish networks with statutory and voluntary organisations. Work collaboratively between designated national centres to ensure a streamlined service. 2.
Knowledge, Training and Experience They will gain specialist knowledge and expertise to assess, plan, evaluate and implement care interventions for patients, requesting support if required. Working to agreed policy and procedure to implement care. Maintains up to date knowledge in their specialist field of practice, ensuring they meet professional requirements. Utilises opportunity for continuing development in clinical and theoretical skills to provide up to date patient care.
Work towards completion of the non-medical prescribing programme. Obtain informed consent by communicating complex information regarding the management of care, including: benefits, risks, alternatives, compliance and lifestyle advice. Utilises clinical expertise, knowledge and skills to take a detailed and comprehensive nursing and medical history, and from this information to be able to plan, implement care and evaluate care. Responsible for maintaining professional competence and standards to deliver care and services within sphere of practice and clinical expertise.
Be involved in, as part of a team, the development and update of guidelines of policies with the support of the consultants and multi-disciplinary team. Deliver programmes of care according to clinical need within agreed pathways and protocols. Provides ward based support and knowledge for patients and staff. Utilises professional knowledge to assess, plan, evaluate and implement care interventions for respiratory patients.
Utilises knowledge enabling appropriate referrals are made to ensure effective and timely discharge of patients. Utilises knowledge and skills to avoid hospital admission. Follows evidenced based treatment plans/pathways working to agreed policy and procedure to implement care. Supports the use of equipment or digital technology such as mobile phones, apps, web-based tools.
Maintains up to date knowledge in their field of practice, ensuring they meet professional and speciality requirements. Work towards supporting and working collaboratively with the respiratory nurse specialist lead. Analytical Skills Monitors patients concordance against treatments and with support, interpretation of specialist information from test results and clinical findings. Takes appropriate action and informs appropriate personnel of any changes to their conditions able to appropriately respond to any emergency situation within patients home.
Takes remedial action in resolving problems associated with the day to day running of the service. Monitors patients health and well-being, and informs appropriate personnel of any changes to their condition, in collaboration with the patient. The post holder will assist the Respiratory Nurse Specialist, assuming responsibility for delegated roles 4. Planning and Organisational Skills Provides specialist patient care to an allocated workload, for the span of duty Assists the Lead Specialist Nurse in the implementation of corporate and nursing objectives Plans care for patients, organising appointments and treatments Responsible for managing and prioritising own workload against the needs of patients and service requirements Delivers care interventions and specialist advice in a variety of settings within the hospital, patients homes and peripheral hospitals.
5. Physical Skills Undertakes advanced clinical skills which may include; Spirometry interpretation Physical examination Chest examination Chest radiography interpretation Phlebotomy Assisting right heart catheterisation studies and delivering Nitric oxide. Responsibilities 6. Responsibilities for Patient/Client Care Responsible for providing direct specialist care to a defined group of patients Responsible for ensuring the nursing care standards are upheld for the span of duty Enabling patients to meet their activities of daily living requirements 7.
Responsibilities for Policy and Service Development The post holder is responsible for following trust policy and procedures. The post holder assists in the development and implementation of local policies, in conjunction with the department manager Responsible for adhering to the NMC professional code of conduct 8. Responsibilities for Financial and Physical Resources Responsible for ensuring that appropriate supplies are available to support the delivery of direct patient care, and enabling team members to carry out their duties Responsible for ensuring equipment within the working environment is maintained and in working order Is proficient in the use of specialist equipment 9. Responsibilities for Staff/HR/Leadership/Training The post holder will support other staff members to ensure they are competent to care for respiratory patients.
This will include training, teaching and education sessions to multi-professional groups, patients and carers to ensure the safe delivery of treatment plans. Participates in the supervision and mentorship of students and junior staff. Ensures all patients, visitors and staff are treated with dignity and respect. 10.
Responsibilities for Information Resources Responsible for providing accurate and timely records on patient care and performance using paper and IT based systems, in accordance with trust policy. Maintains accurate documentation relating to patient treatment. 11. Responsibilities for Research and Development Participates in audits, and research-based studies to evaluate the effectiveness of care interventions.
Promotes benchmarking and the dissemination of best practice. Responsible for ensuring practice is up to date, utilising evidence from national policy and research. 12. Freedom to Act The post holder plans their workload, using guidance, policies in accordance with their professional code of conduct, and in accordance to the needs of the patient group The post holder is required to use their own judgement, to define day to day work priorities and initiate treatments affecting patient care Effort and Environment 13.
Physical Effort The post holder will have direct patient contact, delivering direct care in a variety of settings in accordance with the need of patients. 14. Mental Effort Responsible for producing accurate patient records. Deliver and maintain digital technology used during patient care.
15. Emotional Effort The post holder is required to support a caseload of patients, through a range of treatments and experiences. Many of which can be highly distressing and challenging. .
16. Working Conditions The post holder will work in a variety of settings to deliver care, which may include the acute trust, patients own homes and other healthcare departments, according to the needs of the patient
