REGISTERED NURSE OR REGISTERED PSYCHIATRIC NURSE CASE MANAGER (CM) - MENTAL HEALTH

At Cognito Health, we seek a dedicated Registered Nurse Case Manager to join our mental health team. As a Case Manager, you will play a crucial role in supporting our clients through their journey to mental wellness alongside a team of Nurse Practitioners, CBT therapists, and psychiatrists. Your responsibilities will include client intake assessments, lab/ECG review, and case management for our programs focused on anxiety, depression, insomnia, and ADHD.

QUALIFICATIONS AND SKILLS:

  • Valid RN license in British Columbia, Canada
  • Bachelor of Science in Nursing (BScN) preferred
  • Experience in psychiatric or mental health nursing is an asset
  • Strong interpersonal and communication skills
  • Ability to work collaboratively within a multidisciplinary team
  • Critical thinking and problem-solving abilities
  • Compassionate and patient-centered approach to care

Responsibilities:

  • Intake Appointments: Conduct thorough client intake assessments under the supervision of our nurse practitioner lead/co-lead, gathering comprehensive histories and introducing clients to our Cognito program.
  • Lab Review and Communication: Review laboratory and ECG results, communicating findings effectively to clients.
  • ADHD Scale Review: Assess ADHD rating scales in collaboration with Nurse Practitioners.
  • Case Management: Provide comprehensive case management for clients discharged or escalated due to urgent concerns, collaborating with multidisciplinary teams for discharge planning and post-care support.
