$7,500 hiring bonus for eligible external candidates that meet required qualifications and conditions for payment.
The Primary Care Manager RN provides care management, care coordination, skilled care and services for at-risk frail elder patients with complex medical and psychosocial issues. Population-based disease management approaches and application of clinical protocols are integral to the program. This position is a core member of the patient centered PACE interdisciplinary team (IDT). The goals of care management are to assure a systematic approach to the coordination of health services for patients along the care continuum in order to improve the patient experience, contain health expenditures and improve quality. Clinical Care Managers assist providers in effectively identifying needs and coordinating access to appropriate care and services in an effort to improve or maintain the patient’s functional status and independence.

REQUIRED QUALIFICATIONS:
- Upon hire: Oregon Registered Nurse License
- Within 30 days of hire: National Provider BLS - American Heart Association
- 2 years RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience
PREFERRED QUALIFICATIONS:
- Bachelor’s Degree in Nursing
- Upon hire: Current recognition by a national professional organization in care management, geriatrics or hospice
