Description:

An exciting opportunity has arisen to join a new team operating in the community across the London Borough of Brent.
We are seeking an experienced outreach practitioner to support a multidisciplinary team to help engage with the homeless population who struggle with mental health issues within the borough of Brent. The team uses holistic assertive outreach techniques to support service users to access mental health services and to secure long-term accommodation.
This role is situated within a new team that targets rough sleepers who are isolated and are experiencing high levels of distress. This team will support by providing a reconnection service for those service users with no housing, health, or social links to re-establish links with family/support networks, working closely with third sector reconnection services, as well as foreign embassies to facilitate timely repatriation.
The post holder will use their specialist foundation knowledge and skills in rough sleepers to support assessment, care planning, implementing and evaluating the case and treatment of clients.

Responsibilities:

Team
To work as a member of the multidisciplinary team which provides a range of specialist services to rough sleepers attending the service/unit.
To support the Rough Sleepers Mental Health Practitioners with data entry and administrative work, ensuring clinical records are updated.
To ensure an understanding and implementation of clinical governance.
To ensuring that all work is performed in accordance with Trust policies and procedures, including the Serious Incident policy.

To ensure concerns are raised and escalated appropriately and outreach worker is able to link in with main teams where needed to seek guidance/hand over.

  • To be familiar with all legislation and policies that relate to the team and apply correctly in practice. This will include all housing legislation and government homelessness initiatives in addition to national and local mental health policies and directives.
