Description:
The purpose of this post is to utilise the patient administration and operational systems in the Trust to actively validate, track and manage the referral to treatment processes in a patients pathway and 18 week target. To escalate pathways to RTT team lead which are: - Delayed due to insufficient clinical information to progress the patients pathway. Pathways that require an event or a scheduled event to be brought forward in line with their 18 week pathway. Regular themes of errors in data entry against the patient pathways Patients which are potentially long waiters The pathway validation will be sourced from a variety of data sets, such as PTLs and KPIs.
There may also be Ad Hoc queries or sampling of patients data. There may also be additional queries from the services around RTT rules or system useage. Input and retrieve information from the departmental and/or Trust computer systems in accordance with departmental guidelines and information governance. Maintain records of validation in line with established data capture processes Assist in validation audits and planning To ensure confidentiality and security of data in accordance with organisational requirements, and in line with the Data Protection Act Use patient notes, existing databases, and the Hospital PAS and Pathology system to formulate pathway data on individual patients, making judgements and assessments on pathways to correctly reflect patients waiting times.
To work with a problem solving approach in order to identify solutions resolving daily issues as they arise and escalate where appropriate. To raise any issues of information not being available or missing as appropriate. To actively support the RTT Manager in identifying, reporting and resolving departmental issues in order to achieve a high standard of quality and completeness of RTT data To participate in planned training events and to attend all mandatory training sessions as requested To work with and assist in upskilling new staff as required To collect additional data on an ad-hoc basis as required To record accurately the reasons for patient breaches Liaise with team lead and senior management regarding significant changes to patient pathways and escalate where appropriate to relevant departments. To plan and organise own work schedule, demonstrating good time management and ensuring anticipated difficulties in meeting deadlines are promptly reported To develop and maintain effective working relationships with other staff in the departments and the Trust, ensuring that a professional and seamless service is provided.
