In April 2024, the Boards of St. Mary’s General and Grand River Hospitals both independently voted in favour of a voluntary merger of the Hospitals to form a new hospital organization. This evolution of our partnership will occur alongside our joint redevelopment work, as we strive to build a new acute care campus in the community. We welcome talent to both St. Mary’s General Hospital and Grand River Hospital that are ready to be part of an exciting, generational change for hospital care in Waterloo Region.
Please Note: New employees must provide documentation/proof of full COVID-19 vaccination status, a 2-step TB screening, as well as proof of immunity to measles, mumps, rubella, and varicella (chickenpox) prior to their start date at St. Mary’s General Hospital. Staff will require clearance from Employee Health and Wellness before they are able to begin any position within SMGH or GRH.
Position Summary :
SCOPE (Seamless Care Optimizing the Patient Experience) is a quality improvement collaboration between hospitals and solo primary care providers (PCPs) located in the Kitchener Waterloo area. The SCOPE service is a virtual interprofessional team that facilitates greater access to hospital- based interdisciplinary resources and other specialist care. SCOPE aims to optimize access to primary and specialty care services while helping to reduce avoidable emergency department (ED) visits and hospital re-admissions for patients in need.
The SCOPE Social Work Navigator supports the SCOPE interprofessional team, the PCP, and the patient to navigate the regional psychosocial health services ecosystem system to improve access to specialized care and social health services.
POSITION REQUIREMENTS:
- Masters of Social Work degree
- Membership in the Ontario College of Social Workers and Social Services Workers
- Experience working with equity-deserving populations with complex medical needs required
- Minimum of 3 years of experience within acute care and/or primary/community care preferred
- Bilingual considered an asset
- Experience working within an Interprofessional team environment
- Ability to work independently and autonomously required
- Must be flexible and able to adapt to changing environments and travel outside of hospital and within community
- Excellent interpersonal and decision-making skills
- Excellent organizational skills
- Ability to adapt to a changing environment and assume responsibility for own learning
- Demonstrated strong leadership skills and abilities
- Computer literacy required -Database management skills preferred
- Working knowledge of eligibility requirements for municipal/provincial employment and income supports and how to access such services
- Knowledge of formal and informal community services and resources
- Working knowledge of the Mental Health Act, Health Care Consent Act, Child & Family Services Act and any other relevant legislation
- Understanding of psycho-social functioning
- Ability to diffuse conflict and foster collaborative working relationships
- Demonstrated ability to organize, prioritize, problem solve and respond to competing, unscheduled demands
- Experience and skill in complex decision-making processes, advancing best practice initiatives, and exploration of innovative models of care
- Excellent interpersonal, written and verbal communication, organization, and time management skills
- Self-directed, organized, and diplomatic
- Demonstrated ability to function independently and as part of a multidisciplinary team
- Experience with quality improvement an asset. Skilled at providing virtual (phone) care
- Good computer skills
- Excellent attendance, punctuality, and work performance record.
