Description:

Clinical and Professional: Deliver and participate in the clinical care of service users referred into the team. Offer expert clinical advice for service users, carers, staff and other professionals. Prioritise workload and clinical interventions as appropriate ensuring effective use of time. To be professionally and legally accountable for all work undertaken to practice at an advanced level of professional autonomy and accountability that is within Trust policies and the Nursing and Midwifery Code of Conduct.
To appropriately assess, examine, investigate, diagnose and treat service users with both acute and chronic conditions, resulting in the safe management and appropriate referral or discharge of service users based on specialist body of knowledge. To use independent non-medical prescribing skills in the initiation and evaluation of treatment plans. Ensure service users are informed where appropriate and consent is gained prior to treatment, investigation and management as appropriate. Where a service user does not have capacity, have knowledge of the capacity act, best interest decisions, LPA, court of protection.
Is able to identify the need for appropriate diagnostic tests, interpret the results and act on any results requiring clinical intervention or management. To identify and intervene proactively where circumstances contribute to an unsafe environment for service users, staff or relatives and escalate these to the health and safety team/safeguarding team. To share knowledge and expertise, acting as an expert resource to others across primary and secondary care as appropriate. Contribute to the development and improvement of systems and processes that facilitate patient flow, facilitating timely discharge and preventing hospital admission.
To be able to understand and apply the legal frameworks which underpin Safeguarding of adults from identification of risks and vulnerabilities, to making appropriate timely referrals and supporting staff team in this process. Must have completed the Independent Non-Medical Prescribing course and registration with the NMC, undertake non-medical prescribing in accordance with the Trust non-medical prescribing policy and maintain evidence and competence of this qualification. Adhere to CPD requirements of the Trust in relation to maintaining evidence of on-going competence. To facilitate the Designated Prescribing Practitioner (DPP) role for trainee prescribers.
To maintain up to date service user records in accordance with Trust policy and professional bodies standards, including referrals, risk assessments and outcomes. To demonstrate safe and advanced competence in highly complex and intricate decision making, assessment and the management of clinical needs within the specialist area of practice. To provide advice on service user care to the wider MDT taking into account all aspects of clinical Governance and act as a specialist resource within the working area. Promote and adhere to the principles and practice of the Trusts clinical governance framework.
Contribute to the review and development of clinical service user pathways and the development of the service, ensuring our services are safe. Lead on the review and implementation of appropriate NICE guidance. Will support MDT teams with clinical/therapy competencies. To ensure all practice is evidence based and supported by current literature / research.
To ensure that clinical and non-clinical risk is identified and dealt with appropriately. To report to the Local Division management team any risk that cannot be managed at an acceptable level without external support. To provide verbal or written reports as requested by other departments or the Local Division management team as required. To maintain the high standards in clinical expertise that this role requires.
The post holder will work closely with the Consultant Psychiatrist for their specialist area of practice and other lead professionals within the Division. Leadership/Management Be aware of and keep up to date with advances in nursing and treatments linked to the role. Carry out staff appraisals/PACE. Assess effectiveness of care and service delivery through peer review, clinical governance, quality assurance and formal evaluation to help implement improvements where necessary.
In partnerships with other clinical teams collaborate on improving the quality of health care and respond to local and national policies and initiatives as appropriate. To comply with the requirement of the Mental Health Act (1983) and Mental Capacity Act (2005) working within their Code of Practice Guidelines. Support and participate in shared learning across the trust and wider organisation. To take a role in teaching, providing expert professional advice and education to patients, carers and colleagues.
To identify training needs and participate in developing and delivering such training within the service and wider Trust. To oversee and take an active lead on organising risk management strategy meetings. To take a lead on the promotion and organisation of reflective practice sessions and complex case discussions. Have negotiation and conflict management skills.
To contribute to the development of Trust Policies and Local Division Guidelines. To adhere to Trust Policies reporting to the Local Division management team any failures in adherence to Trust Policy. To identify and lead in areas for service development in your area of responsibility. To actively promote the role of the Advanced Nurse Practitioner across the Trust.
Ensuring all key targets are maintained, undertaking core duties, as well as helping develop the roles of the workers in the clinical team. Taking a lead role in ensuring the service is effective and meets best practice benchmarks outlined in local and strategic objectives. Provide and ensure excellent leadership and communication structures are in place across the service. To critically appraise, evaluate and apply knowledge of theoretical frameworks and research evidence within a specialist area of practice.
To act as a role model to staff by demonstrating leadership and expertise within the service and Trust. To critically evaluate and review innovations and developments relevant to the area of work. To offer and participate in regular individual and group systematic supervision. To provide strategic clinical leadership in relation to an area of specialism using the most up to date and evidence based theory.
To be trained in root cause analysis and lead investigations on clinical incidents and complaints when required. Support the Deputy Chief Operating Officer in delivering required objectives and developing Local Division nursing strategy including involvement in clinical governance at a senior level; to participate in on-call Rota and the duties that this entails. Quality, Research & Audit Undertake and participate in appropriate audit, quality assurance programmes and research as directed. Continually evaluate and audit the practice of self and others, selecting and applying a wide range of valid and reliable approaches and methods that are appropriate to the needs and context.
To maintain, monitor and present data relating to Advanced Practice while continuously evaluating the effectiveness of the role. Participate in clinical research as necessary whilst ensuring adherence to ethical practice. Monitor quality and performance of staff within the team. Assess effectiveness of care and service delivery through peer review, clinical governance, quality assurance and formal evaluation to help implement improvements where necessary
