Senior CHC Nurse Assessor

Managerial Responsibilities To deputise for the CHC Operational Lead Nurse in their absence. To manage the junior nurse assessors in the team. To organise meetings with multi-disciplinary teams (MDT) within the service and key staff members whenever required. To support junior staff members with their caseloads and capacity management.
To work collaboratively with the CHC Operational Lead Nurse in managing the financial budget and control and adhering to good clinical practice at all times. To ensure all data collection are up to date and provided in a timely manner to the CHC Operational Lead Nurse and the CCG for performance review as required. To respond to complaints in accordance to the Trust guidelines and policies, ensuring any actions required are carried out. To be responsible for ensuring effective multidisciplinary team working and the development of positive multi-agency working.
To ensure that all individual records are holistic, accurate, individualised and updated regularly. To monitor all aspects of adverse events and implement changes to prevent reoccurrence and improve individual care. To have the knowledge and experience to challenge traditional clinical practices to ensure that evidence based best practice is identified and implemented
