Description:

This is an exciting opportunity to join our department as a band 7 pharmacist. We support our staff to complete a postgraduate diploma and/ or independent prescribing whilst developing the necessary skillset to grow into a senior pharmacist.
The post holder will join a team of specialist and generalist clinical pharmacists, medicines optimisation technicians and pharmacy assistants in delivering high quality, safe and effective care to patients and service users. You will undertake ward based clinical pharmacy activities in addition to six monthly specialist workstream rotations. These areas include and are not limited to: medicines safety, pre-operative assessment, drugs and therapeutic committee, spinal rehabilitation medicine, acute pain, chronic pain clinics, clinical trials and research, metabolic bone disease and rheumatology, homecare, complex long-term bone and soft-tissue infection management as well as post-operative care and high dependency/critical care.
This is a full time role entailing 37.5 hours per week and includes weekend and on-call commitment on a rota basis.
We are looking for an enthusiastic, self-motivated, forward thinking and pro-active clinically orientated pharmacist to join our progressive team at RNOH. There is extensive scope for the successful applicant to develop on both a personal and professional level through exposure to all departmental activities, contributing towards portfolio building and the opportunity for further roles in the future. The post holder will be required to possess good communication and interpersonal skills, be able to work effectively in busy environments both independently and within a team. Computer skills are essential. Ideally, the post holder should have previous NHS experience and be working towards or completed a clinical pharmacy diploma.

Medicines optimisation service provision
Support senior pharmacy managers in the delivery of a high quality, patient-focused medicines optimisation service for inpatients, and where relevant, outpatients; thereby enhancing the quality of the patient experience and outcomes for the patient.
Apply core clinical pharmacy skills in order to plan, manage, monitor and review therapeutic programmes for all patients.
Identify and manage patients’ pharmaceutical needs and make appropriate referrals.
Professionally screen/check prescriptions, thus taking professional responsibility, as required by legislation.
Integrate research evidence into practice
Medicines Assurance and Governance service provision
Support the senior pharmacy managers in the provision of medicines-related assurance to RNOH, its patients and various other external organisations.
Support the senior pharmacy managers in the provision of medicines-related governance through the various structures within RNOH including RNOH Drugs and Therapeutics Committee, Medication Safety Committee, Antimicrobial Stewardship Committee etc.
Contribute to medicines evaluations and critical appraisals to medicines
Participate in medicines-related incident reviews, investigations and complaints
Contribute towards medicines-related root cause analysis as appropriate.
General Service Provision • Review prescriptions for their clinical appropriateness, safety and legality.
Input prescription information using the Pharmacy Computer system.
Request, dispense and supply drugs to patients.
Provide evaluated pharmaceutical advice and information, to all health care professionals and patients of the Trust using on-line data bases and primary sources.
Have a working knowledge of pharmacy stock control and ordering systems
