Description:

An exciting opportunity has arisen for a highly motivated and enthusiastic pharmacist to join the pharmacy team at Nottinghamshire Healthcare NHS Foundation Trust.
We are seeking an experienced pharmacist to join our inclusive, dynamic and supportive team based at Lings Bar Hospital. Successful applicants will be given the opportunity and support to learn, develop and succeed (including leadership and postgraduate learning opportunities).
You will be responsible for the delivery of high quality, proactive and patient centred clinical pharmacy services in a range of specialities across the rehabilitation pathway. Lings Bar Hospital is a 72 bed inpatient unit. The role provides a wonderful opportunity, working in collaboration, to prepare, empower and support our adult patients across Nottinghamshire to achieve their goals for their onward health journey.
Fixed term, up to full time Band 7 position (up to 1.0 WTE, up to 37.5 hours per week) with Emergency Duty Commitment. No weekends/Bank Holidays.
The post is permanent and up full time, although part-time and job share would be considered. Consideration will also be given to those returning to practice. The successful candidate may also be required to travel to other Trust sites as service needs require.
To ensure the safe, effective and economic use of medicines in collaboration with the patient and multidisciplinary teams across various specialities.
Provide pharmaceutical input at multidisciplinary team reviews and ward rounds, take drug histories and devise patient-specific, collaborative pharmaceutical care plans where necessary.
Develop close working relationships with medical and nursing staff to ensure that pharmacists are integral members of the clinical team.
To develop and participate in a programme of in-house training for medical and nursing colleagues, pharmacists (including mentorship for those undertaking the clinical diploma), trainee pharmacy students, pharmacy technicians and assistants.
Liaise and work in partnership with the Care Groups to provide expertise on medication/prescribing related audits across services and facilitate continuous quality improvement.
To champion antimicrobial stewardship.
